The Basics

Phalloplasty is the surgical (or sometimes non-surgical) modification of the penis, often aimed at increasing size. The term "Penoplasty" can also be used interchangeably with "Phalloplasty."

Non-Surgical Girth Procedures

More popularly known as girth enhancement, non-surgical girth phalloplasty involves the use of injectable fillers to increase the width and circumference of the penile shaft. This procedure requires no general anaesthesia and is typically done within an hour (hence non-surgical). Typically, a patient will need 1-to-3 days of recovery, and can resume sex and/or masturbation in 7-to-14 days (depending on the filler and physician protocol). Popular fillers include Hyaluronic Acid (HA), PMMA, and Ellanse. You can find summarized information regarding each popular filler on the Inforgraphic found on this page.

Hyaluronic Acid is a naturally occurring substance that is already found in your skin. It helps keep skin plump and hydrated. HA fillers are typically soft and gel-like. The results are temporary, lasting 12 to 18 months before the body begins to gradually absorb/break down the filler, and 18-24 months (or longer) for complete absorption.

Most HA fillers are infused with lidocaine to help minimize discomfort during and after treatment. Popular Brands Include: Juvéderm and Restylane.  www.americanboardcosmeticsurgery.org

Pros: Best Safety Profile among all Fillers available; naturally occurring in the body, formulated for use in cosmetic medicine. Reversible to some extent (varies). Provides instant girth. Very good value per CC. Offered by the majority of PhalloBoards Sponsors.

Cons: Non-permanency requiring future top-offs.

Popularly known as Ellansè, this filler is injected into the skin to lift, sculpt and restore lost volume associated with skin aging. Its principle component is Poly-Caprolactone (PCL) microspheres – a bio-resorbable soft medical polymer. The PCL is suspended in a gel carrier made of carboxymethylcellulose (CMC). Unlike hyaluronic acid fillers, which lifts because of its properties as a gel, Ellanse works by stimulating your body to produce collagen. The results last 2 – 4years.

Pros: Creates collagen for girth which tends to be firmer. Long-lasting (as much as 4-5 years depending on the type of Ellansè injected). Most Non-American Sponsors offer Ellansè.

Cons: Not available in the U.S. (FDA Approval pending as of March 2021). Due to the aforementioned pending approval, long-term outlook is not well known.

Polymethylmethacrylate (PMMA) is a synthetic, biocompatible substance that has been used in medicine for much of the last century. In dermal fillers, PMMA takes the form of a “microsphere” or tiny ball, that remains beneath the skin indefinitely to provide continued support. PMMA fillers will also contain collagen, a naturally occurring substance in the skin that provides structure and firmness.

Popular Brands Include: Linnea Safe and Bellafill®.

Pros: The only permanent injectable filler recommended by the PhalloBoards. Long history of patient reviews showing strong long-term outlook.

Cons: Permanency can also be an issue if a patient has an undesirable result, since this procedure is difficult to reverse.

Renuva is an alternative to autologous fat transfer injections. Renuva is a synthetic fat filler which can be used to create volume.

Popular Brands Include: Renuva®.

Pros: Unlike Free Fat Transfer (FFT), this procedure is non-surgical and won't require the harvesting of fat from the patient's body.

Cons: Results aren't as easily reversible when compared to other temporary fillers like Hyaluronic Acid (HA).

Glans enhancement is the augmentation of the penile glans, typically done through dermal filler injections. Hyaluronic Acid (HA) is the most common filler used in this procedure. As of 2024, because of the inherent challenges posed by the anatomy, there is little in way of advancements in this area, and gains are often minimal and temporary.

Surgical Procedures

Also known as ligamentolysis, this surgery involves severing the suspensory ligament from the pubic bone, which will allow the penis to hang lower, thus achieving length. This is often coupled with a stretching regimen post-op.

Results seem to vary patient-to-patient, largely dependent on each individual's unique anatomy. Flaccid gains are usually more commonly reported, however erect gains can be achievable with a commitment to the Doctor's prescribed post-op protocol.

A condition where the excess scrotal skin is attached to the shaft, which can lead to numerous issues. These issues include (but are not limited to) discomfort during sex, inaibility to use certain devices adequately (e.g. penis pumps), prohibitive of proper condom use, visual aesthetics, and other unforeseen complications. There is a surgery that can correct this issue by removing the excess skin (i.e. penoscrotal or scrotal webbing reduction surgery).

Also known as dermal graft matrices, these are tissue grafts from a donor (typically human or animal tissue) that can be used to thicken the shaft of the penis. Popular brands include Surgimend, Alloderm, and Belladerm.

A surgical procedure that extracts fat from a donor site that can be used as a filler injection to increase width and girth.

Also known as the "fat pad lipo", this surgical procedure extracts fat from the pubic area in order to improve the appearance. Can make both the flaccid and erect lengths appear longer.

Protocol Enhancements

The P-Long protocol is a natural penile enhancement technology that uses platelet-derived growth factors and the AFFIRM nitric oxide boosting supplement to accelerate the effects of traction and suction on a healthy man’s penis. P-Long was created and tested by Dr. Judson Brandeis, a urologist and sexual medicine researcher. Over six months, men can expect an average increase in length of almost an inch, girth of nearly half an inch, and an improvement in erectile function. The P-Long protocol has no side effects, and men can continue treatment well beyond six months. No synthetic materials are required, and the penile growth appears proportional and natural. A large and growing network of P-Long certified doctors can provide this service closer to home than Dr. Brandeis’s office in Northern California at a price well below other penile enhancement techniques. Jelqing is a manual penile exercise technique that involves using a lubricated hand to firmly grasp the flaccid penis at the base, with an OK-grip formed by the index finger and thumb. The hand is then moved in a rhythmic motion towards the glans (head) of the penis, applying gentle pressure. The idea behind jelqing is to create a controlled force that stimulates blood flow and stretches the tissues within the penis.

Although lacking scientific or medical validation, anecdotal evidence suggests that some individuals have reported improvements in both penile length and girth. Without medical supervision there is risk for injury, both temporary and permanent.

Penis pumping is a method used to temporarily increase the size and firmness of the penis. It involves using a cylindrical tube that fits over the penis and creating a vacuum inside the tube, which draws blood into the penis and causes an erection. Penis pumps come in both manual or electric versions.

Although lacking scientific or medical validation, anecdotal evidence suggests that some individuals have reported improvements in both penile length and girth. Without medical supervision there is risk for injury, both temporary and permanent.

Hanging is a traction-based penile elongation method, a technique that utilizes weights to induce controlled tension on the penis, aiming to promote lengthening. Typically weights are tied to the penis and worn for a set time.

Although lacking scientific or medical validation, anecdotal evidence suggests that some individuals have reported improvements in both penile length and girth. Without medical supervision there is risk for injury, both temporary and permanent.

Stretching, also referred to as "manual stretching," involves applying traction to the penis by gently pulling it in various directions for a predetermined duration and intensity. This technique is commonly employed as a warm-up before engaging in other penis enlargement exercises or protocols.

Although lacking scientific or medical validation, anecdotal evidence suggests that some individuals have reported improvements in both penile length and girth. Without medical supervision there is risk for injury, both temporary and permanent.

Clamping is an advanced technique that is cautioned against for beginners due to its complexity and potential risks. It involves the application of a cable clamp at the base of the penis to create a constriction, thereby retaining blood flow and inducing prolonged engorgement.

Although lacking scientific or medical validation, anecdotal evidence suggests that some individuals have reported improvements in both penile length and girth. Without medical supervision there is risk for injury, both temporary and permanent.
