That's the way to do it! 2014-2019 is a great window of study and I'm glad you've put the time in.

As for your questions:

Answer:
I don't know that there is a general consensus on a specific HA brand, but products like Voluma, Juvaderm, and Restaylne stand out as the most commonly used for penile application(s). I suspect the "erection too soft" may be a result of over-filling, which could make any volume filler palpable and thus "squishier" or "softer." HA in my opinion is best used as either a spot-filler -OR- girth enhancer in low-to-moderate volumes.

2. Answer:
There are Clinics and Practitioners popping up left & right now that this procedure-type is proving its efficacy over time. That's also why I'm careful to vet potential Sponsors (and have even turned down some in recent months, despite it being a potential revenue stream) as I take pride in the legitimacy of this site and those Sponsoring it --> this isn't a standardized or universally accepted medical field, making it far more susceptible to quacks, hacks, and scam artists.

Fortunately, HA injections represent the safest technique (only rivaled by FFT) on the market and most newly advertising clinics are going to vary on quality/workmanship and not so much safety/malpractice. That said, I'd still try to find a practitioner with some sort of presence or relevance online. If Thailand is a consideration, would you be willing to travel to Australia or is that too far/expensive at this stage? I only ask because Dr. Oates with CALIBRE specialize in HA and have had stellar results (can be found here and on PB 2.0, photos and all).

HA PROS: Injected with something that naturally occurs in your body and is about as safe as a filler can get.
HA PROS: Is both temporary and immediately reversible should you be dissatisfied with the results, which continues to boost its safety profile.
HA PROS: Can provide both a visual and tangible increase in size.

HA CONS: Temporary fillers require top-offs periodically and indefinitely to maintain desired size. Despite it re-absorption varying patient-to-patient, you could easily see the necessity of injections once every couple/few years to maintain a set girth. This is true for all temporary fillers (HA, Ellanse, Radiesse, and Fat), so it's more of a "reality" than it is a "negative."
HA CONS: Is not ideal for large volume gains. Due to its volume-filler nature, it's best used used in low-to-moderate amounts to prevent a squishy or palpable penis. This again is more a "reality" than a "negative" since gains as "small" as 0.25 inches are tangibly/noticeably thicker, and I could see gains as high as 0.5 - 0.75 inches working on some units. Simply compare something as much as 0.25 inches thicker to your current penis and see how crazy it is to chase whole inches of girth.
HA CONS: Since any physician performing elective, cosmetic medicine can employ its use, there are many who aren't necessarily trained or skilled in the proper injection techniques and may not provide the best aesthetic, natural results (pardon this crude analogy, but someone who does nails for a living wont necessarily give you a great hair cut just because they are allowed to use clippers & scissors).

I personally give HA a thumbs up because in my opinion, represents the safest means to enhancing one's girth. It isn't the most ideal, nor the longest-lasting, but it sure has the safety profile going. Even fat transfers, which were traditionally what I regarded as the safest means of enlargement, can't compete with HA. This is because fat extraction can be somewhat invasive and HA doesn't have that problem at all.

I sense that you are dealing with a lot and I genuinely hope you and anyone in a similar situation end up with the best case scenario. I've read all your posts on the old forum and am well acquainted with your past, so pardon me if my rebuke was harsh. I just wanted to to tone down the misplaced alarmist rhetoric. I also didn't appreciate the suggestion that anyone is being attacked because they disagree with you, or choose to defend the merits of PMMA, and I hope we can move past those kinds of remarks.

This forum is intended to be a place for all former, current, and prospective patients of phalloplasty. I intentionally named a forum category "the Patient Support Group" so that men knew this was an anonymous online sanctuary to connect, share, vent, and learn. Presenting your side of the coin is not only welcomed, but encouraged. Half of the Moderation team were patients of botched procedures. I brought on Dr. Solomon as a Sponsor largely due to his work in fixing procedures that went wrong. I've spent years exchanging emails with lurkers (members who don't post publicly) about ways to fix their penile issues, who to see, and what to expect. Trust me, all sides of the coin are welcome here.

If I didn't want "negative" or "critical" reviews of PMMA I'd simply ban or delete the topics. Furthermore, I have no financial incentive in promoting PMMA as I have other Sponsors who perform alternative procedures like Ellanse, HA, fat transfer, and Surgimend. For full disclosure, I did have PMMA performed on me over 8 years ago and a few follow-up rounds in the years that followed. I've had zero instances of complication since.

As for glossing over negative reviews, we'll have to agree to disagree. What's truly glossed over is how the current generation of phalloplasty candidates are mostly unaware of what came before them and how much worse things were in the industry. Up until this past decade, men were discussing either dermal graft matrices (still viable with proper technique and graft), silicone oil/gel, PLGA scaffolds, fat transfer, and a rigid silicone implant. The success rates were abysmal and the nature of the complications associated were severe (i.e. length retraction due to scar formation, nerve damage and loss of sensation, impotence, infection, and necrosis). By comparison, PMMA (and similarly Ellanse) have exhibited considerably better complication rates, and the nature of those complications are relatively minor compared to complications of old. This may explain why there appears to be a "glossing over" of less-than-stellar PMMA reviews--> they're infrequent and seldom ever as severe as to warrant a threat to penile function and health.

That said, discussions of potential complications and the means to address them are always a relevant discussion topic, just as they have been since this forum's inception. I hope Dr. C continues to chime in, I'm also very curious about the idea that granulomas can "disappear." Is it possible they could disappear indefinitely or is there an inevitability of their return?

Greetings gents, as some of you know, I've been a vocal advocate of Ellanse for a while now. For those who don't, it is my belief, after thorough research, that Ellanse is the best filler option for the penis available today. For safety reasons, and the fact that I'm already endowed with good girth, I've resisted the call for a filler procedure. Aside from wanting a bit more length, I'm pretty happy with my size. However, one thing that I'd really like is a longer flaccid hang, as I'm very active and I'm often in situations where I need to wear compression-type pants. Due to the advent of recent developments (namely Dr. Morganstern's clinic offering Ellanse in the U.S., applied by a board-certified urologist, Dr. Carney), my concerns over safety have diminished. Given my age (44), I'll be running into more mature women (larger vaginas), so a bit thicker is ok. This post will be the first in a series of posts that will chronicle my experience with Dr. Morganstern's clinic and Dr. Carney's technique for Ellanse in the penis for penile girth. Below and attached are my pre-op stats. The procedure is tomorrow. I will not be taking immediate post-op pics, as I believe that to be a fruitless endeavor, given that girth at that stage is mostly swelling. I will resume picture-taking following the one-week mark. Off to Atlanta.

There\'s actually no improvements for the past 10 or even more years on this field. Things like PMMA for PE already were existing in early 00\'s in Brazil (and keep in mind that at the present time PMMA exists for 30 years). Also, I\'ve somewhere read about using of HA for penile glans enhancement like 15 years ago. If you want more (not only about fillers procedure): the ligament cutting procedure were first performed for cosmetic reasons by Chinese surgeon (forgot his name) in 1990, moreover, first mention of this procedure (not for cosmetic reasons) was in early 1970\'s.Now you see that there\'s not so much changes for the last 10-20 years?

Jelquing is a p.o.s. People get soft glans, venous leakage and PD because of this shit. As well as 90% of manual PE routines/excercises is a total shit. If you want to add length, then you need to try manual stretching, penile extenders and hanging. All of these techniques working very badly, but that\'s only that we have in 2018 (it fucking sucks). If you want to add girth, I\'d recommend you to look at vac pumps (if you want temporary gain) or better looking at options like HA, Ellanse, PMMA, because despite of all the limitations of fillers, it\'s still better than torturing your dick for many years just for a 0.5\" gain.

I was actually thinking about this for different reasons. But it seems like every 6 ml might equal about 1/4\" if your EL is 6.

Another thing to consider is the additional thickness that you will have over your tunica. You can approximate this with a circle formula (I know the penile circumference may be more oval). For example, if you went from a 5\" EG to 5.5 EG, the change in radius (amount of filler over the tunica) is just a little more than 1/16\". If you went from 4.25 EG to 6 EG, that goes up to 0.28\" which is more substantial. For voluma this would be an issue, but it may be different for pmma and ellanse. Both subtle and glam82 have over a half inch EG gain with ellanse and its hard as before. Maybe SO can comment as he\'s had the most dramatic change (saw pics on thurnders, but can\'t find his stats).

Pmma isn\'t the only option. Some of us are considering a filler called Ellanse. It\'s a stimulatory filler, which means that the process of having it in you incites collagen production so that your own body continues to create your desired growth. From most accounts, it seems to be safer and much more stable than pmma. There is an innovative doctor on this board called Dr. Oates, from Australia, who is offering Ellanse to patients looking to safely augment their penile girth. A member here, Subtle77, has undergone the treatment and reports good results after 6 months

Thanks, Subtle77. Your progres report is the most important to me, as it features the stimulatory filler which I have a feeling is the best \"ml for ml\" for girth enhancement. I believe you\'re the only member here with an Ellanse progress report. I wish more members here would use and report on Ellanse, so we can really get a good sense of this material. In the meantime, thank you for your contribution.

The questions for me, as I move closer to me pre-filler penile lengthening goal, will be \"where, who and how much?\"

I\'m curious what sex has been like with your Ellanse enhancement.

Hi Aleksk,

Ellanse is a lot more expensive than HA (here in Australia). Stimulatory fillers are generally more risky than HA. Only HA is able to be dissolved - so if the worst possible thing happens, such as getting injected into the penile artery, only with HA can it be reversed.

Stimulatory fillers have much more tendency to be lumpy. I have used Ellanse (but much less than HA) over about 4 yrs. It tends to feel harder to start with but over all patients are happy. I have had 1 patient with an auto-immune reaction and granuloma (lump) formation. It was in the face - I really dont think someone would be happy with that in the penis. But women are much more likely to have that issue.

We use Voluma. It tends to last years, definitely more than 1 year - but we are waiting to see how long.

Yes there are a variety of reasons why it is not a better known treatment. Urologists generally dont want to do enlargements and are not familiar with fillers. Most cosmetic doctors dont really want to do enlargements either. There are the fears of doing something new - the risks to the patient and even moreso how that will affect your otherwise successful career. I think it will change slowly. Also those who are doing it, generally want to build up their own practice before telling the competition about it.
