With about 60% loss of skin on my shaft, I\'d say about 60% of the PMMA is gone. I\'m certain that skin prevents retraction, especially PMMA infused skin. I learned from experience, this forum, and from Avanti Derma that most who have PMMA see their Flaccid hang increase. I previously had two separate wounds measured by the nurses at 8.25cm x 3.25cm and 5.25cm x 2.25cm. The doctor cut off dead and dying skin, including between the two wounds, creating one large wound measuring 13.25cm x 1.25cm. How could my wounds become smaller when more tissue is removed, if the overall size isn\'t decreased? When I pull my glans and stretch it out, abracadabra the dimensions of the wound increase to about 5cm longitudinal length, and if I push down at the base it opens even larger. Right now my Dick looks like Balls with a mushroom cap and almost no shaft, since it\'s all retracted inside. Skin would certainly provide some structure to keep the rubber band-like ligament from pulling my shaft so close to my body. His logic is that if we reduced the size of my Balls it would make my Dick look longer (the skin he wants to remove from the base transitions into my scrotum.

Think about Iong walks or other physical exercise to improve mental health and get you through this low piont. it\'s great the way you are informing other members of what you are going through as others here will be in the same position too.

Do you think if the cysist had been removed earlier it could have stoped the the infection spreading or do you think it was only a matter of time before it spread.

I understand now, Mike.
I have been reading over the Darkstaff\'s log, he was 4 inches at one time. He got all his length back and recovered fully. There is also case I linked to in other thread of someone having a full Removal 2 years after PMMA. He looked terrible at first, again, he recovered fully.

I wonder how many other\'s have problems to come.

When Dr. C drained/aspirated a cyst the fluid was clear, with no visible indicator (or smell) of infection. Though the sample wasn\'t taken to the lab, so I can\'t be certain there was no infection. I think the infection began when the skin was compromised, following a period of peeling of layers of skin which lead to \"pitting\" over the underlying abscesses.

I think early and regular treatment of the cysts and later the infection might have prevented or reduced the negative outcome. Another major problem with PMMA is that too few US doctors have experience with it, or dealing with negative outcomes. This causes a lot of time (and money) wasted looking for doctors, which is especially bad if you\'re in the middle of a necrotic infection where you lose more of your Dick to decay every day that passes. One of the best things members of this board can provide each other is names of doctors experienced in dealing with penile reconstructive surgery and patients who have had dermal fillers injected in their penis.

I know from experience that gaining Girth is relatively easy, compared to gaining length which is extremely hard. I know that my pre-PMMA Girth gains and my post PMMA Girth gains (not due to PMMA) were huge, and that PMMA and silicone microdrops were not worth the risk. The \"good\" results for dermal fillers still end up with irregularities and detectable hard spots beneath the skin. But some people will do it regardless; like body builders who choose to use steroids despite the known risks, when they could build muscle naturally instead. I shook my head when I read the post of a newer member who stated he wasn\'t gaining from using a Bathmate, but didn\'t like all the extra Foreskin he was growing...I was like WTF did he think the new skin was if not gains.

Dr. C used a sterile syringe to drain the cyst.

I\'m quite shocked reading your experience. Do you know the reason for the injection? Is it from PMMA or something else? Can you please describe your Clamping routine and how you used the pge1? Best Regards and alle the best for your recovery

Well the actual infection started after the skin was compromised (small pits formed after peeling). The pits allowed bacteria from the outside to get inside. By compromising the skin, the encapsulation surrounding the PMMA was possibly also compromised. Some of the exposed PMMA spheres inside possibly became contaminated. The existing cyst(s) also made good location(s) for an infection to take hold; as infection is a common outcome with chronic/untreated cysts.

I\'m not sure what exactly caused the peeling and flaking, and it was widespread on the penis and not just localized to the cyst areas. It is possible the skin wasn\'t getting adequate blood supply due to the filler and hypertrophied skin from PE activities. The diuretic (water pill) I began taking for blood pressure is also a prime candidate. Or it could be combination of both factors. The peeling began within two weeks of beginning the diuretic (hydrochlorothiazide), and by the end of the month I had two large infected ulcers and sent pictures to Dr. C for advice.

A story that sticks in my memory is that the silicone enhanced (monster) of internet fame named \"Greg\" had an infection caused by his scrotum becoming punctured in the course of outdoor activity and had to have all of his filler removed and reconstructive surgery. On an episode of botched the doctors commented how when a breast implant breaks through the skin, it becomes contaminated by the environment, leading to infection and requiring Removal.

I don\'t have the motivation to write a lot about Clamping and PGE1 today. I made long posts about those activities in the past, but a lot was \"bulldozed\" when the related sub-forum on non-surgical enlargement was wiped out and reset to zero. I might be motivated to write about my experiences with those items again in the future.

I think the most likely thing is that you have 2 separate issues. The first is the cyst which you seem to be predisposed to. The second is I think the silicone got infected due to PGE 1 injections. I think the pre existing cyst then the ideal environment for the infection to spread. How you describe the skin flaking sounds very similar to how Miracle\'s issue with silicone started. I don\'t think PMMA would cause for skin necorsis like you\'ve experienced as it\'s injected deeper, whilst the silicone can migrate into the skin. I can\'t see it being due to blood pressure medication.

About a year ago someone posted on here that they\'d read of guys with silicone enhancement getting infected after using PGE 1. Anytime you pierce the skin you run the risk of allowing bacteria in.

I also recall the chatter about silicone enhanced guys becoming infected from PGE1 injections. As far as deeply injected PMMA vs shallow silicone microdrops - PGE1 is injected deeply (into the corpora cavernosa). With my previous Girth I used .5 Inch insulin needles and had to push them in deeply. The Urologist also trained me to enter at the base below the PMMA ridges (on both sides), which avoided a lot of the PMMA \"armor\" (build-up of hard PMMA encapsulated and granulous tissue especially notable near the base due to retraction at the time of initial treatment). I certainly agree that the \"pitting\" or perforation from the skin peeling wasn\'t extremely deep, so the bacteria from those openings would have had initial access to silicone injected skin, and later the cyst (a good breeding ground for infections).

When Dr. C aspirated fluid from that cyst in June 2018, he also gave me my second and final 25cc silicone in the scrotum. Another thing you read about (and see evidence of) is migration of silicone and PMMA between the penis and scrotum, especially early on before things become encapsulated, so I wonder if the last silicone injection set off an immune response within months. Perhaps the answer to the suspicion of migration was in my own signature block. In June I recorded a large jump in Erect Girth. That month I also found myself with a lot of new Foreskin as well, and all the problems that go with that for the first time (smega, having to retract it during erections, etc).

When I got my PMMA treatment, irregularities (ridges, lumps, pits, voids, etc) were a fact of life, and from what I read they still are for many. As I mentioned before, when I arrived at AD I stated that I wanted to be one treatment and done, and was corrected (before we had even started) that I would almost certainly have to go back for a second (touch-up) treatment. Sure enough, I didn\'t like my initial results at all especially when women would point to, manipulate, and call out the irregularities. There was no way I could deny that I had injections done to most. I went back to TJ for a touch up within months. Then I went back for another, and another, that included two sessions of silicone microdrops (aka smooth finish), CaHA filler in my glans (negligible effect) and had two 25cc injections in my scrotum (the last mentioned above in summer of 2018).

Hopefully new members or visitors here also visit the \"PE\" boards (e.g. Thundersplace) for more comprehensive information on non-surgical options (most of the content on this board was removed). I saw another apparently new member declare that since his penis returned to normal after removing the Penis Pump it doesn\'t work. It would be like picking up a weight one time in a gym and not seeing massive muscle growth, therefore declaring weightlifting ineffective. If people would read they would learn that method will take months to years, and mostly benefit Girth with much less (if any) length gain.

With the first surgery that cleaned and debrided the wound, the surgeon removed some skin that he felt would die, but had the idea that he was going to be able to keep 40-50% of my skin. Instead, it continues to erode - a slow painful death. The doctor says if the blood supply is good enough the skin will live, otherwise it won\'t. In fact, he stated multiple times his intent to remove more skin from the base (as it merges into the scrotum), even though this skin is surviving and not \"distressed.\" From an article I read showing reconstructive surgeries after filler, the penis skin had been removed around the entire circumference down to the buck\'s fascia, from below the glans down to the scrotum, before skin grafts were applied. It seems to me like this method would create a more uniform result, vs. grafts fused to abnormally enlarged/hypertrophied leftover skin.

Now when I pull on the glans to open the wound and apply a wet gauze dressing, it only extends about 3.5 inches from my body, with the rest of the shaft buried inside. I asked my surgeon if he was planning on bringing a Urologist to the surgery, to help with the buried penis shaft that has adhered to interior skin, and if I should meet with the other doctor ahead of the next surgery. I told him I was concerned that if they don\'t use a clamp, manual traction, or other method (vasodilator to cause an Erection?), the skin grafts he would apply would be too small (for a 2-3 Inch penis). I was disheartened to hear that he plans to apply grafts and close it \"as is,\" and that I could seek treatment and surgery from a Urologist for the buried shaft at a later date. Seems like a huge waste of time and money, since another surgeon will have to release the suspensory ligament and detach the shaft fused to surrounding skin. If successful the next surgeon would expose more shaft length, and this will require additional skin grafts to cover and close the wound. I made an appointment in May (after my planned April surgery) with a Urologist that specializes in correction of buried penis, and other reconstructive type surgeries.

Meanwhile, my scrotal skin was peeling severely during the dressing change this morning. The tape used to hold gauze squares on came off with a layer of skin stuck to it, and more bits were peeling in surrounding areas. This might portend more doom to come due to the scrotal silicone injections.

The surgeon removed all of the remaining skin on the shaft from the base to the glans, and extracted some of the buried shaft before applying a graft to the Buck\'s fascia surrounding the shaft. I\'ve been sitting in the hospital since last week with a wound vac and Catheter, so nobody has really seen how well the graft is doing. So with the amount he pulled out I\'ve doubled from 2 inches to 4 inches. He\'s been giving me doses of viagra during healing so that has actually increased pain with nocturnal erections. Depending on how things go with my \"unwrapped\" erections in a week or two, I will decide whether to keep the appointment with the Urologist for correction of buried penis.

Sorry to read this. Feel for you Fat Mike. I have only had PMMA, but worry when reading things like this. Things like this seem more common with silicone. Regarding your cysts. Have you had that always even before doing anything? Do you know why you have them? You also mentioned granulomas. Sometimes this is just a lump and do nothing. I am a bit confused after reading the thread. Can granulomas from PMMA give Necrosis years later?

I\'m still healing with weeks to go (at least). Since all of my skin was removed down to Buck\'s fascia before a skin graft was applied, I no longer have any Dermal Filler in my penis or a cyst. To create an even result with the graft and reduce the buried penis effect, the surgeon also removed the peno-scrotal webbing. My scrotum skin had been enlarged due to the silicone filler and years of pumping and PGE1, and the portions that webbed into the penis had merged with formerly huge Girth there.

The cyst(s) was probably caused by PMMA. On my last visit in 2018 where I had an additional 25cc of silicone added to my scrotum, Dr. C identified one cyst, but it\'s possible there was a second. Cysts are great breeding grounds for infections, and they can become infected over time on their own. If the skin becomes compromised this can accelerate the process. Foreign bodies such as implants and Dermal Filler also increase the risk of infection if the skin becomes compromised. Based upon unusually fast growth in my penis Girth in the months that followed my last visit to Dr. C (originally thought to be all from PGE1), it seems likely that some silicone migrated to my penis. Additionally, I previously had also had silicone microdrops as a touch up on two occasions following PMMA. Additionally, I had a history of cysts elsewhere on my body (two different locations), so I was very likely at higher risk (predisposed). Perhaps that should be part of a pre-PMMA questionaire (medical history) in order to screen people more likely to have complications: \"have you ever had a cyst?\"

There were two initial locations that became compromised/perforated following my skin peeling like a sun burn - probable allergic reaction to silicone. One of the infected ulcers was directly over the former cyst, and the second may have also been over an unidentified/unconfirmed cyst. If you read the log of Miracle 8x7 he also had skin peeling leading to infection, that he\'s confident occurred as a reaction to silicone. My scrotum has repeatedly peeled in the same fashion; however, my scrotum skin heals very fast (apparently this is normal/common).

Regarding the so-called granuloma: that was my doctor\'s assessment. However, according to Wikipedia: in medical practice, doctors occasionally use the term \"granuloma\" in its more literal meaning: \"a small Nodule\". Since a small Nodule can represent anything from a harmless nevus to a malignant tumor, this usage of the term is not very specific. in my experience and from what I\'ve read nodules, ridges, and other irregularities are common with PMMA. From reading many progress reports and my own personal experience you can live with these indefinitely as long as there is no pain/discomfort, swelling, redness, infection, etc. I read that complications from granulomas requiring surgical intervention are relatively rare . I didn\'t have any pain or discomfort, just dissatisfaction from the PMMA irregularities, which is why I sought out the touch-up rounds.

I measured Erect today...very pitiful length - 4.25 inches NBPEL x 6.5 Inch EG. Bone pressed there\'s more length, but it doesn\'t make me feel any better that inches of my former length are buried within my body. I also know that 6.5 Inch Girth is a lot compared to average, but to me it feels small (I was 7 Inch Girth pre-PMMA, PGE1, etc).

Still healing...no sex or masturbation for months (since March) weighs on your quality of life
