Hey Dr. Rupeka!

1) for an uncut penis, how long after the procedure do you recommend waiting before doing any Hanging with weight around the glans with the Foreskin retracted?

2)do you have to do the Bellafill bovine collagen skin sensativity test on every new patient?

3) can you do Bellafill on a guy already has superficially placed HA in the skin?

Hey!

1. I want my patients to have their penis as extended as much as possible directly after the procedure. If you are thinking of doing Hanging or gentle stretching with something (like a noose) around the glans, with the Foreskin retracted, I’m okay with you doing that immediately after as long as the noose is directly behind the glans and not around any of the area that the filler was placed.

2. I have never done a bovine collagen sensitivity test on anyone (and I’m the number 1 provider of Bellafill by volume in the world). The skin test is going to go away in the US in Q2 this year and has already been removed in Canada. Possible sensitively to the bovine collagen sort of went away when they started becoming more systematic with where the sourced the bovine collagen from.

3. Yes, but this is an important caveat. I have seen two new patients to me this week already that had previously placed superficial HA done. If I inject a penis like that which how I normally inject, you won’t really notice much of a difference with the PMMA because that superficial HA “shell” is still going to surround what I added (I hope that makes sense lol). In some of these cases, I layer my PMMA on top of the fascial layers but under the superficial HA that is there already. This would add some structure to that layer and it’s nice because the hydrophilic HA will help keep the Bellafill hydrated as the bovine collagen gel gets degraded (meaning that even though it is more superficial, there should be less chance of nodules because of the hydration…similar to when we inject it in areas of the body where there is subcutaneous fat). It would just really depend on what the overall goal is with adding the PMMA.

Thanks Dr. Rupeka, all of these answers help alleviate alot of my worries about the procedure. Last questions!

1)For an uncircumcised penis, do you fill with the Foreskin retracted or forward over the glans? Is one way safer than the other?

2) can the procedure be done if i have a curve to the left? Can the procedure fix or visually improve the concave side of the curve?

3)And what can be done to decrease the risk of the filler leaking out of the deep space into the skin during and after the procedure?

1. For an uncircumcised penis, I inject with the Foreskin retracted that way I can still watch my cannula for any movement more superficial under the area just proximal to the glans.

2. Sure, curvature is fine (most have some). I inject in the Flaccid state. I would never say that this procedure is performed to correct a concavity or convexity. Yes, we can place more product on one side or the other to create a volume deterrent, but you are doing this procedure to enhance the Girth as the primary endpoint.

3. Well, there is obviously no guarantee as these procedures are blind procedures and done by feel. I would do my best with respect to performing the procedure as intended and then you would do your best following aftercare as directed.

My penis is thicker at the base and tapers towards the glans, and this bothers me a little. Is it possible to make it a bit thicker and with a uniform circumference using fillers, even if I'm not Circumcised?

Yes, usually we can place more product one way or another to add Girth more towards the base or glans. I would say that most of my patients are going for a little taper toward the glans, but obviously this would be based on whatever the patient’s aesthetic is. Hopefully the filler should have some linear (lateral) spread through the space that I am injecting it into, so it will even out somewhat, but I would try to place more product in that direction and then you would position your aftercare plan towards working it in that direction as well.
