I might have been unclear before. Those concerns I mentioned in my original post were precisely that: they were only concerns; however, it was not something I had experienced myself. Necrosis is a possible complication soon after surgery, but I did read studies where this happened to a small number of participants, and it was successfully resolved. Another complication that is much more serious would be infection caused by rejection of the dermal fat graft.

As for the dermal fat grafts dying, this is something that a few members here reported happening to them, but it seems to have been brought on after 1-2 decades by an addiction to nicotine. Whether or not dermal fat grafts can survive indefinitely is not something I have been able to find information on. If I were to make a very uneducated guess, I think eventually they might simply reduce so much in size that they simply become unnoticeable, rather than needing to be surgically removed. However, the member on the forum that had them the longest mentioned having them for over two decades without reduction in size.

As for penile retraction due to delaying using penile traction devices, I have read many clinics write that it is almost impossible to happen with the new surgical techniques. Some clinics even write that it is not necessary to use a penile traction device, but in this case most people would have no length gains from the ligamentolysis. At the same time however, the surgeons surely know that many people give up using penile traction devices, and then it is a comfort to know that the penis will not retract.

I bought the Phallosan Forte Plus, and I must say that it is not a simple device, but it seems to have more ways to use it compared to other penile traction devices. I believe it is a good idea to buy one a few weeks before having ligamentolysis done, in order to get comfortable using the device, because there is obviously a learning curve.

There are many medical tourism agencies that help people organize their trips to Thailand and South Korea, and these agencies are usually run by Westerners in countries such as Australia. I think it might be a good option to cooperate with them, rather than cooperating directly with those clinics and hospitals in Thailand and South Korea. If someone wants to travel by themselves though, it might not necessarily be hard. I think there are not so much language barriers as there are cultural barriers.

Finally I want to add one thing about selective dorsal neurectomy, which is not directly related to phalloplasty, but many clinics in Thailand and South Korea will offer it as part of a combination procedure: lengthening, widening and selective dorsal neurectomy. One clinic in South Korea even claims that a huge number of Westerners come to them, and most of them will choice this combination procedure.

In Asia many people do not like the idea of pharmaceutical treatment for most ailments, and it is why selective dorsal neurectomy has become so common, whereas in the USA and Europe it is thought that there should be no reason to recommend anything other than a pharmaceutical treatment. Dapoxetine has been shown to be very effective, after all. It is just that many Asians are very uncomfortable with the idea of pills in general.

As I understand it, selective dorsal neurectomy can be very minor, so even people who do not experience premature ejaculation choose to perform it to "last a little bit longer". My surgeon told me that he can do a minor one if I have little to no problem with premature ejaculation, but I would still like to increase the time to ejaculation, and I choose this minor one.

Because it is such a common procedure in South Korea, its safety has been studied very thoroughly, and supposedly it is a very safe procedure. The risk of complications albeit extremely rare include possible nerve damage and nerve pain. So to summarize this point: anyone traveling to Thailand or South Korea for phalloplasty will likely see selective dorsal neurectomy offered as part of a "package deal".

Thank You for asking these questions. these are great. Lets clarify them.

1. I do not get erect to roll. I roll VERY often. I do get erect a few times a day though and that is recommended by most doctors I consult with.
1.1 I do use an ok grip AT ALL. I feel it moves to much material around. When you heat things up like I do, we dont want to do that we only want to shape and mold and move as needed where needed.

2. I recommend different stretchers at different times. For immediately after a procedure, during the " AFTERCARE " phase of roughly 1 week, with a very very strong emphasis on the crucial first 3 days we only want devices that attach to the head/glans ONLY and or just behind it like a noose. THATS IT ! after this first week there are some other devices to be used, Still not a sleeve though at this time. I would a sleeves and vac stretcher is roughly at 3 week to 1 month mark.

2.2. there have been some guys on here that have modified their phallosans and other vac devices and think they have done well. I refuse to fight and argue over this so I just say ok and shut up. But here is the bottom line: DO NOT TOUCH YOUR DAMN SHAFT FOR AT LEAST A WEEK. lol. If you do in anyway, you are doing wrong.


NOTE: I have a patent pending product that is a recreation of a product that was discontinued by its creator. I will offer this once i get the first viable products in from the manf. Just a heads up, but this isnt a quick process.

@Joytoy please feel free to ask more and fire back. I think these answers are what people need to see and hear.

Hey all,

Been waiting patiently to finally upload my progress report. I'm now on day three post procedure.
Firstly, thank you to all the guys who have contributed on here over the years, and more recently Dr Oats, AudienceK and Texas that have responded to recent questions of mine. Thanks for the advice guys.

My Journey started years and years ago, probably 20 years ago, when I first wanted to increase my length. I started with pumps, and one of the first generation noose style stretching devices. I either didn't give these devices the proper commitment or was using them incorrectly, and hurt myself in the process. I soon retired my commitment, and the devices were turffed.
It wasn't until the last few years I caught wind of the cosmetic industry and fillers being a viable option. Surgical interventions being risky and expensive were not really for me.
This year I decided I was going to take the plunge and commit once again to making this a reality.

This year, about 6 weeks ago, I purchased the Phallosan forte. It was a nice improvement on the noose style stretching devices, one that I hadn't tried either. I think it's a great device and noticed a slight improvement in length pretty quickly. I didn’t take any measurements unfortunately but thought that when erect there was a slight improvement in length. Very minimal, but still, I could see the potential. For the record I won't be using this device in the near future as it will get in the way of the filler and recovery. Maybe in the future, down the track. Will have to re-visit this option another time.

From my own research earlier this year I decided I wanted to use Ellanse. The two reasons being longevity of the product and (thanks to Phalloboard contributors) the firmer more natural feeling of the end result. I then narrowed down my search to two clinics in Sydney that offered Ellanse, Calibre clinic and Contour Clinics. By the time I was ready to book Calibre's practicing Dr has left so I booked in to see Dr Wall at Contour Clinics. Dr Danh was also a contender working out of Melbourne's Calibre Clinic but I decided to leave that as a second option if my consult with Dr Wall didn't eventuate or go as planned. I say this because I get the feeling some doctors are either not experienced with Ellanse or just oppose it. Dr Tall in Brisbane emailed me after an enquiry advising me against it. So I was open to the fact I may not be accepted as a patient for Ellanse. My option then would be to get a second opinion with Dr Danh in Melbourne.

Fortunately Dr Wall was fantastic. Welcoming, friendly, and agreed with me after I mentioned I had done my research, a big credit to Phalloboards here. I wasn't new to the topic and I understood the risks involved. I'm also in the healthcare industry so this may have helped. Understanding human physiology and the healing process is a benefit. This is still a risky procedure if not cared for appropriately post procedure.
I had a local anaesthetic injected to the base of the penis so I didn't feel a thing, the process was over in about 20-30 minutes. I received 15 mls straight up, using a cannula. I was shown how to massage the shaft post care and was given a script for 5 days antibiotics prophylaxis. I also asked for a course of cialis too (28 days). My member was wrapped up in stretch wrap and given aftercare info that explained exactly what I had to do.

I think the after care is CRITICAL for this to go well. There are so many variables that could influence the final result. Every penis is different in so many ways. Reading as much from this forum is key as information I have picked up here has made a valuable difference.
So I had to drive home post procedure and my car is quite low, I was nervous about the 30 minute drive with my penis at a horizontal level and not facing down. I was doing all that I could to push against the seat and raise my body vertically up. I wish I had caught the train now. I had that day off work, but unfortunately had to return by lunchtime the next day. Fortunately my job is fairly dynamic in regards to movement of my body whilst at work, consisting of seated and standing, I'm not seated for long periods of time and I can get to a bathroom fairly frequently to attend aftercare.
I had ordered a noose style stretcher but it hadn't arrived (and still hasn't) by the time I had the procedure done. I was a bit anxious about my plan from here. I was going to buy another stretcher from an adult shop but was reluctant to pay 4 times the price when there was one in the post.
However what I did discover next, accidently, has changed everything for me. I found a new, but unused, shoelace and decided to experiment as I had no other options to keep my penis stretched and facing down. I carefully placed the shoelace in the crease around the glands and tied a little knot that was neither too tight or too loose, situated in the little space on the underside of the glands as the edges of the glands meet. Its the perfect spot for a little knot. I then and wrapped the shoelace once around my thigh and tied it together below my knee. This worked so well I have been able to wear it pretty much all day long without much discomfort. It's completely and easily adjustable. I either lifted up my trouser leg and untied, or visited the toilet for a few minutes to re-adjust. To tight and I knew very quickly, whilst too loose and the glands slowly slipped through the noose, but it didn't hurt. I will upload a video showing how easy this is too put together.
At nights it became too much while trying to sleep and removed it. I haven't slept much the last few nights, but that's a choice, I've been trying to stay up late and stay vertical, or least seated with my keg lower than my pelvis. I slept with many pillows and an additional foam mattress, forming a V shape that I slept prone and with my penis in the middle, in the canyon I had formed with all the pillows etc. I have massaged my penis 3-4 times a day for at least 5 minutes. I used wet face cloths in a sink of hot water, alternating the cloths and keeping my penis warm while massaging. I used a vitamin C serum to assist with massaging- I'm unsure if this will help but I have it here and thought, why not. Vitamin C is required for collagen production and works on face skin so I thought I'd give it a go here too. I will continue using that during the 12 weeks that Ellanse stimulates collagen growth. I am also taking Vitamin C supplements, extra strength Zinc, and collagen powders and tablets. I'm also committing to eating more fruits and veggies, especially oranges and bananas. Almonds also have copper which s good for collagen neogenesis so will be eating them everyday for the next 12 weeks also.

I will continue my massaging, stretching, eating well and will visit Dr Wall for a review in about 5 weeks. So far I'm pretty stoked with the results. I'm sure I may have forgotten some valuable information here but will commit to updating this post as time goes on.

Hey all,
Fairly new here and loving the advise and honesty of all that contribute.
I recently bought the Phallosan forte yes I found it slightly cumbersome but then I think most devices are by there very nature. I think I’ve tried them all over the years. The Phallosan, I think is definitely the easiest to wear for long periods of time, which is obviously important.
By the way I will upload a video or photos later. Right now it’s on and I don’t want to disturb him while he’s busy working.
1. The sleeve that is attached to the bell, that the company advise to roll down and then over the rim of the bell, I find cumbersome.
2. What I do is, similarly to when applying the glans protection cover, I place my pointer finger and middle finger of both hands into the sleeve pull wide apart and let my penis drop into the space. I’ve also had the idea of trying this step without the bell, applying it next. I will test this and update.
3. Once penis is in the sleeve, let the area of sleeve closest to the top of penis, near bell opening retract and grip the shaft and slowly inch your finger out.
4. Some overlapping can occur at the base as you try to remove your fingers, which I suspect I’ll get better at preventing overtime. This needs to be corrected as I assume would add unnecessary additional force which could interrupt blood blow and cause pain. Carefully pinch the sleeve and adjust to leave a flat sleeve.
5. Now resume with the pumping and suctioning as normal.

I found this method far easier and produced a better sustainable suction.

I am pre PE procedure, although am planning to have done soon. Not yet sure of my plan for post procedure,

@Oslo I personally would not use a phallosan as a hanger. I would get a cheap noose from Zen. You may want to add Locking Bead to it like I did. get them from hobby lobby. Start hanging with that and then use the phallosan after your hang to hold your length - as a stretcher. As you get more into hanging, then you can gran another hanger for the mix and get a clamp from PE hanger, Male Hanger, Total Man. Total Man may be the cheapest.
For using a noose - YES. start slow. Go 10 to 15 min at a low weight like 2lbs. or even 1lb the first day just to get used to things. Use the one and two pounds for a few days and get used to feelings, the usages, the tightness, the time , etc. Get used to how your glans will look and feel. It will get cold but you dont have to worry; But, if it gets purple ( and it will from time to time ) dont worry either just dont let it stay that way. Take it all off, take a couple minutes rest then hit it again. Then go up to 3lbs after a few days. A

Again, In My Personal Opinion, there is no need to get over 5lbs on a noose. I dont go over 3. I just go for more time if i can. However, every so often, you can bump the weight for like one session for the shock. but remember the scar tissue story.


@honested YES. my erections are no longer up at all. but I knew that going into this and it never really bothered. Frankly my wife didn't seem to really notice or care either. Its bigger and thats what i wanted. haha The sex has been just fine and no real change for us. Im still early on in the healing, So I guide him in there as opposed to the old day of just poking it in.

Thanks @texas ! So can I use the phallosan pump and modify by having weights or or I need to use a noose with weights?

And I’m guessing start with 15min and gradually go up to 2 hrs with breaks every 15min?

@Oslo It's not that it is better, it is that it is different - it pulls different; and so yes, lol, in a sense a bit better. But it is BEST to use all of these things. The doctors ( all of them ) are not experts in these devices nor do they have time tested, hands on, experience with them and how they all function. That's where some of us come in; to help each other out. What they ( The Doctors ) do know is: Hanging and Stretching ( both Pre & Post ) surgery has shown benefits.
To elaborate further a bit on the doctors and the devices ( lets call it speculation for legal safety ): these doctors have to be somewhat careful with what and how they recommend certain activities as well as certain devices. FOR EXAMPLE: The green noose from Zen, which is a great little device, is now no longer given out by a "particular" office due to the amount of complaints of uncomfortableness as well as self described misuse. And when I say Self Described - what that means is: unbeknownst to the describer, they are describe misuse in some way. ( IE: leaving on too long ) So therefore - that is to much risk.

OK OSLO, let's talk Phallosan. Which I hate btw, but I've had one; So i can discuss. I only hate it because its fucking cumbersome and ripped the damn thing 3 times. But it did work great as a stretcher and it was very concealable ( better than a lot ). If you are using only this, then YES you need to be incorporating some hanging into your life - but no need to go heavy ( see my previous posts ). When hanging - there are a few devices that are great, rotate them all. Think of it like going to a gym - Do you do ONE chest exercise and go home or do you do a few ?
For time, for up to the two hours per day. start with 30min and see how you feel. then go 45 and then an hour and so on. I think you will find that at the lower end of the weight poundage you can hit the 2hr per day pretty fast, just don't jump right in. lol. Get comfortable. As I mentioned before; for clamp style devices that go on the shaft, you will be able to do more weight, but try to not go to heave. For head/glans clamp style devices or Nooses you will not be able to go super heavy and shouldn't, the pull is enough, go for time. And for Vac Devices; Some people can go heavy and some stayed light ( I always got blisters ) - my advice is to keep it moderate to light and go for time and take it out frequently to get blood.

So hanging weights is better than vac (phallosan) ? How long you wear the weights, like 2 hrs a day? Did Dr.Carney mention to do this , sicne they mentioned the autoextender

I’m right now doing phallosan but not seeing significant results, I’m like around 5 months in about 8-12hrs a day and still nothing special to boast about

Certain thing mentioned here I’m ver skeptical Especialy the taking out the lumps.

But for the stretching, I’m curious on since if you get filler in glans and use the phallosan, will it mess up glans?

I do have filler in place. I have had 30ml of Ellanse.
They did not give me a device. I provided my own.
For the record, at all, of the phallosan. There are many better and easier devices out there.
For pumping. I Do not plan on trying that for a while. But really want to see how much further in the tube I can get. Lol.

I would call them again and also send an email regarding what you are wanting. I doubt it was intentional that they didn’t get back to you .

Sounds like a common response to overtraining.

Starting slower, even with some 10-20minutes manual routine for example, could be a good idea to conditionate your penis. Saw you already came to that conclusion as well.

There is a concept often talked about on Thunders Place called "Physiological Indicators," and it's meant to help prevent injury. These indicators are "signs" or "sensations" that something isn't right. Don't over exert, and frankly, if your penis doesn't respond well to an exercise, tone it down or discontinue it altogether. We're all built differently, including our penises, it's important to listen to our body's guidance in this regard.

Many of us are performing these exercises without medical supervision or clearance. This is why being cognizant of your body's reactions are so key to success or failure. Maybe this device and/or exercise isn't the best fit, especially given your description of the situation. Or maybe tone it down considerably, to like 10-15 minutes at a time. I believe 90 minutes is a lot of time if you aren't experienced.

Personally, I don't recommend these exercises without medical clearance or supervision (like for example, after a ligament surgery), and if you are to perform them, start ever-so light & slow. I look to try pumping again soon, and I too will be on the lookout for physiological indicators.

I am new to wearing any type of ADS. Even after just wearing it for around 90 minutes for one session, I find my erection quality greatly suffers for a couple days afterward. I dont have it on a high setting of tension. Its pretty discouraging. Im worndering if anyone else has ever experienced this. Its not uncomfortable for me to wear nor painful at all, My junk just doesent seem to like it.

Before the surgery I was 6.25 inches in length and now two years after removal I'm at 6 inches so there is hope. Lots of manual stretching and I even used a device called phallosan forte which I believe has helped as it keeps the penis stretched all day. Was thinking of getting a mini penis weight system from Zen hanger. So yes there is hope but also a lot of self work involved. Good luck man and let me know if you have any other questions.
