Here\'s the update:

Derm -- looked at wound & as it has recovered from silver nitrate - I watched it like a hawk & applied bactroban, neosporin, celacyn, etc... and he said he really doesn\'t see signs of a biofilm. He read Prof Lemperle\'s email, but he thinks that it might have been hard for Lemperle to know for sure, given that it was only on email. He thought systemic antibiotics didn\'t pay - and topicals were working fine. He said NOT to have anymore surgery or excisions for at least 2-3 months on that site. His office and pathologist cannot do PCR.

Uro -- He\'s the one at the major teaching hospital. He hasn\'t seen me since he\'s booked up solid, but he saw the same pics Lemperle did. He thinks that excision will take care of the problem, and the biofilm won\'t return if it is there (though he hasn\'t seen post silver-nitrate pics since I hate to bog him down with emails & it takes him a while to get back). He didn\'t comment on my PCR question...but I checked with my insurance and the whole thing would be around $300 since I am at my out-of-pocket max already (good lord! It\'s definitely not all penis related, but about half was). To cancel would be $250 and would also piss him off...and he\'s one of the best male urologists around so I don\'t want to do that. I\'m thinking he could do some excisions of the other nodules and address this in person. I wish I could see him before the procedure, but it\'s just impossible with his schedule.

My urine drips again after I finish...which was happening when I had that base Nodule pushing on my urethra that got Kenalog + 5fu and shrunk. It almost seems like it increased in size over the past 3 months. Maybe he can take that out during the procedure...or the ventral Nodule that also pushes on my urethra...so in a way, it\'s still worth doing. I just hope these freaking incisions CLOSE. Most of them did, but we didn\'t use the hyfrecator which can melt PMMA (after the first one didn\'t close). Uro wants to use it...said he\'ll be very selective about just a few blood vessels...not just burn the whole area out... but that\'s easier said than done.

(Edit - I saw a wound care specialist who made things worse & then had the Urologist do the excision. The details are continued here)

Here\'s some more. What do you think? If it seems crazy I did it myself, it\'s just because I have all the sterile equipment and epi and lidocaine. I used to do animal surgery that was actually fairly similar (except we used staples & my suture ss are pathetic - though I could practice a little & probably do just fine). One of my nastiest, deepest incisions healed GREAT just with steri strips. The other ones reacted to the suture material and seem to be delayed or just very atrophic (like an ice pick acne scar). I hope it fills in like yours did. I know this is all early, but since it\'s been going on since March 24 - it\'s making me nuts.





This bottom right one was the biggest mistake since my entry point Nodule here wasn\'t that bad...and the stitch reaction and scar are much worse (at least now at 3 weeks ). Maybe in a few months it will flatten...

Thanks hunkydory...yeah i really hoped this would be over by now so i\'m glad it worked for you. I put the shower head right on there but it didn\'t break it up. The derm used a blade to cut out the yellow stuff last time.

I bought tamanu oil a week ago - I\'ve used it a few times but i have steri strips on some of the spots so it\'s hard to keep it from running onto those and loosing them. I\'ll use it once it\'s all stable.

My uro (who replies to emails in like 10 minutes - it\'s crazy) said \"It\'s not Necrosis- it\'s fibrosis- scarring which is filling in from the base of the wound. Not much to do other than local skin care that maybe the derm can address- if it does not heal this skin may need excision and primary closure but not yet.\"

So... I guess these things have to heal.

Anyway, on the Nodule front - I still feel hard patches of PMMA in there... so they wouldn\'t have softened up with time (at least not 3 months). There\'s a prominent one on the top of my shaft that is invisible to the eye but very palpable.

Regarding scars - I just don\'t want a guy to think I had warts removed or something...and have HPV. I\'ve seen plenty of dicks too but none with holes. Maybe they will fade...but I cannot believe how long it\'s been since I could do anything. I avoided pride since that ends up being a fuckfest. I avoided so much stuff... I keep putting people off who wanted to meet me SINCE MARCH! How long can I have colds and headaches and \"too much work\" ... ugh ... they are gonna think i am the biggest flake but I can\'t do anything - still at least a month away now that the wound is open again.

Oh, and i\'m moving for the first time in 3 years.... i hate moving. It\'s all just too much crap right now. I don\'t even like where I\'m moving & i have to park outdoors in this weird fucked up spot... I\'m rambling, sorry - i am just in a bad mood these days. Moving and a scarred up penis are making me nuts.

Hey Restoration - if you are referring to the white tissue in the open wound I had that and was told no more stitches by the doctor.
My wound opened up 3 days after the surgery. He told me to keep it open and run the shower head on it twice a day to break it up and it worked. It took about 3 weeks. I have a visible line now but no dent. It is hard to say if the line is permanent as I still have hyper pigmentation on my dick from the surgery which is slowly fading. It may just all blend in when I fully recover.

As far as the dent I have had a lot of scars and none have healed with a dent so I bet it doesn\'t; I know when I get overly distressed all these little things completely overwhelm me (I am talking about the dent now, which is not the least bit unnatural or unattractive). I have seen a lot of dicks in my day and none are identical and they all have their quirks and I have never thought twice about them - I am usually more aware about the person they are attached to. The down side of this site is that we end up overly identifying with our dicks sometimes.

If the white tissue pic is a recent one, use the shower technique it works and you have more control over the situation than when it is stitched shut.

Lastly, I use Tamanu Oil on my dick to soften the scar tissue from the stitches. I would forget to put it on sometime and then I realized you can beat off with it

Well, this time the stitches were in for 10-12 days and the Necrosis is back. So, now it\'s 5 weeks after the Nodule excision...and it\'s still fucked up.

I am going to lose my mind at this point.

The other sites look okay...but not great. The one in my deglove scar is invisible...and the one I did myself is nearly invisible.

I have no idea why my derm used silk Sutures as they are notoriously inflammatory and I was already having a foreign body reaction...now I have redness around all the suture sites with entry holes.

Has anyone here been through anything like this ever? I seriously can\'t deal with any more Dick problems.

Thanks CHL. I was gonna make a whole post about all the microscopic side of things, but I don\'t want to make the info confusing - and I\'m not a pathologist. I met a really nice pathologist sit with me for 45 min and go over everything that she knew about the situation. She\'s never seen PMMA before but she\'s seen a lot of silicone leakage from breast implants and other things, so she said it sort of reminded her of that, just a smaller diameter of the particles. The reaction is similar in terms of how the body walls off the foreign particles (same with anything, even a splinter - silicone just looks like blobs too).

My wound (from above) never closed so I went back to the doc & he debrided and stitched again (now 4 weeks post Removal). I hope the PMMA in the tissue isn\'t affecting the wound closure. He\'s leaving the stitches in 2 weeks this time...which will mean the wound is taking 6 weeks just to close. But, I think by mid - july... I\'ll be normal-ish.

Sure - yeah I thought it was pretty interesting. It also gives a little insight into the nodule issue. Some nodules will respond to kenalog (and simply waiting) because they are just inflammatory - and can last a couple months. Others are not going to go anywhere because they are have too much pmma clumped together. There\'s no way to know unless it\'s taken out - or if you try kenalog/5fu and wait a month and it does nothing...then you can infer it\'s not going to reduce. DMSO may have some effect as well on the inflammation. Red1975 had success with that. I\'m not sure if it would loosen up the PMMA or not & disperse it...especially with all the other tissue there. But, who knows.

With regard to the white dots, I could be more precise. PMMA itself is clear, so you\'re just seeing light passing through where the PMMA is. But, yeah in general - it\'s similar to what you\'d see with artecoll (maybe artefill too - it\'s hard to say exactly):

I had two nodules removed by a doc in the usa - more details are here. One healed already, but one is just going very slow. The nodules were rock hard and not responding to Kenalog or 5fu. Under the microscope, they turned out to be 80% PMMA, so it\'s not a surprise that they never responded to Kenalog.

This is a slice of the Nodule magnified. The blue dots are nuclei of lymphocytes which is normal and leads to collagen building / fibrosis which produces Girth. The white dots are PMMA beads. There are some histocytes and multinucleated giant cells in this image, and other images have more. Each slice is a bit different...but this shows the packed PMMA beads which created the hardness of the Nodule. They should be spread out more, ideally.

Thank you so much for the info! I am glad I know what to expect. I wonder if I went to a Urologist - if it would\'ve worked out better since they know how to put tension on the penis so it stays closed despite nocturnal erections and stuff. I have a few more that I can bring to my uro - but there might not be a difference. My deglove scar was totally sealed by a Urologist. Well, either way, I need to end the celibacy - so maybe I\'ll just wait to heal and have a lumpy penis - and remove those in the fall if DMSO doesn\'t help them.

I had quite a bit of cautery at the end of the procedure to stop the bleeding, which surprised me given how small the area was...and I wondered if that added to the trauma there (red and bruised still). When I\'ve had moles removed & even a lipoma (that was much larger and deeper on my back - my doctor said \"I feel like a brain surgeon\" during that...lol) - I cannot recall any cautery. It just was bloody for a while and I changed the bandages every couple of hours for the first day or 2. They cauterized my deglove scar, but that was a very big scar - like 3.5 inches...and I still have a hypopigmented burn from it (minor - but it\'s there). This Nodule excision was about 5mm I think...but now it looks big.

Well, I\'m glad yours went back to normal...sounds like mine should too.The penis reduction line is great!

Here\'s the scar from the other side - it\'s healing perfectly!

I didn\'t use strips. It seemed like I needed them but was told to keep it open and use the shower head on the white tissue. You are keeping the wound open which is counter intuitive, but the new tissue came from the bottom up. It took about 6 weeks and at first it was a very slow process and then all at once it came together.
I have a whitish line, but the rest of my shaft is still dark from the surgery so I trus it will all come together in the end. It is not noticeable Flaccid, but is a little noticeable hard, but much less than a month ago so I assume it will all come together. The oil is great for softening the scar tissue. I am not worried about it, and if someone asks I am telling them I had a penis reduction as it was just too fucking big before. Most important thing is the get rid of and keep out the white tissue.

Hey - thanks a lot for letting me know. When it was stitched and I posted that pic, I remember you said it would just turn into a line - but when it started spreading out, I was like...oh...crap...especially since the other one is basically healed into a line now. But, that\'s great that it turned into a line for you. Is it noticeable Erect?

I saw your pics at 2 months post. Is this about how long it took to be ok to have sex? I\'m just trying to get a handle on how long my celibacy period will extend... It\'s too bad I was seeing someone (kinda informally) before all this - and I think by now he\'s kinda over it with me since I\'ve been sexually MIA.

Did you use anything to hold it together, like steri strips?

Hi Restoration,
if you look at my thread I had the same hole - I was told that the white stuff is non-viable connective tissue and has to go - and the best way to get rid of it is to stand in the shower and let water hit it. I did this at least twice a day; 3 x\'s if I could. The wound does fill up with regular red tissue and the hole is 100% gone now.

I did have a line of tougher scar tissue left. A friend gave me Tamanu oil to put on it, which worked. The best way to put it on I realized after a few days, was to use it as lube when you jerk off - it makes all the skin on your Dick better.

What is left is a thin white line that is not noticeable when Flaccid and less and less visible as time goes by. There is no difference in height between where the hole was and the rest of my tissue now.

The most important thing is to break up the white tissue as that will keep the healthy tissue out. It looks like you have some white tissue that is dried up in there but I may be wrong. Use the shower a lot, it works.

ps the Tamanu Oil is for once the hole is filled, just use antibiotic ointment until the hole is filled with healthy tissue.
