First off, it is important that you do not underestimate the magnitude of the surgery. Even after doing ligamentolysis alone, it is recommended by all surgeons not to have sex for 8 weeks, but you are doing many procedures at the same time. I do not know what Proud Urology told you about the recovery time?
And did they mention anything about use of a penile traction device after ligamentolysis? As many board members have written, it will be hard to use a penile traction device when combining ligamentolysis with other procedures.
I know that Proud Urology requires you to have circumcision, if you are not already circumcised, so you would not risk prepuce edema that might prolong your recovery time significantly. It seems about half of people who are not circumcised and get dermal graft will have prepuce edema, but in the studies I read everyone recovered after 1-4 months.
I would say it took me about 3 months for the phimosis to be significantly reduced, and now 4 months later my foreskin is what would be considered normal, although still the foreskin is tighter than before. Anyway, it is not relevant information for you because you will be circumcised, whether or not you are already circumcised
As for MegaDerm, I believe it is a bit different from dermal fat graft, because having dermal fat graft inserted requires the penis to be degloved. From what I read, it is different with MegaDerm and AlloDerm, however it might just be that they use different "wording". Somehow the MegaDerm has to be fixed to the tunica, after all.
When implanting dermal fat graft then the penis is degloved and doing this also makes it possible for the suspensory ligament to be incised without ugly scars. The graft then has to be implanted on the dorsal surface of the tunica. In some procedures of AlloDerm I read the dermal graft is somehow "injected" into the penis using a procedure similar to circumcision. Again, I believe this might just be "wordplay", and actually the penis will be degloved to insert the MegaDerm.
Another important thing to ask might be how they prevent penile retraction after the ligamentolysis? Do they use silicone buffer or an adipose tissue flap? Personally, I would also feel a bit uncomfortable with MegaFill, as it seems that it can easily move. What did they say about this post-op care in regard to MegaFill?
I just wonder if it is easy to remove the MegaFill if necessary? Because complications do not seem uncommon at all. Anyway, this is definitely an interesting procedure, but I cannot help but wonder if someone needs all that: glans enlargement? MegaDerm/MegaFill? Ligamentolysis? Selective dorsal neurotomy? Circumcision?
For one thing, do you have premature ejaculation? Do you want flaccid length gains? Do you want circumcision? In retrospect, I think I would just have gotten some filler, and if I was in your situation and in Korea right now, I might have considered only getting MegaFill or HA. MegaFill does seem to require constant massaging for it not to move.
Like I said before, why not have another consultation elsewhere? You could meet with Dr. Kim Jin Hong. Even though some bad things have been written about him, I believe he is by far the most experienced penis surgeon in Korea. However, it does not necessarily mean that he is the best, of course.
