PERCEPTION DRIFT

Observations by Dr. John Leonardo (leonardomedicine.com) that Patients should be mindful of when going in for girth enhancement injections.

Perception drift is a psychological phenomenon where an individual's perception of what is "normal" gradually shifts over time, often due to repeated exposure to small changes.

I present an example of a former patient who publicly vocalized his anger that he spent over $20,000 on nonpermanent filler and it had all degraded after 6 months. $20K was spent not only on the shaft but also Penile and Scrotal Botox along with glans filler. He is lengthier than average and naturally requires more product. He also desired a considerable girth increase.

Each time he returned he complained that it was all gone but the images are quite the contrary. His baseline flaccid images are from April 2024 prior to any treatment by me. That day he received Grotox (Penile Botox) to allow him to hang longer flaccid: This is necessary to prevent contour irregularities after filling. He returned May 2024 for his initial filling. He has topped up September 2024 and May 2025. Each time complaining all the filler had been consumed.

When comparing his baseline before and after Grotox photos to his images prior to his last top up, he is still larger. In addition, there is considerable weight loss as evident in the loose abdominal skin. Despite this, his penis is still thicker and hanging longer.

P-Girth has such a realistic feel/texture that patients don’t realize the product is still present. Each day is their new baseline and their perception drifts as they forget where they started.

Part of this patient’s frustration is he wants a permanent solution. I never promised him permanent filler. Radiesse-HA will degrade over time. There is also swelling immediately after the procedure. Some of that volume is temporary and patients feel they lost filler when everything has settled after a few weeks. I make my patients aware of this:
