Hi guys-

Hoping I can get some support or advice here. I had my first round of 28cc PMMA placed just shy of 5 weeks ago (July 10'th). Initial healing went very well. I was even able to have gentle sex on the 5'th and 7'th day with no issues. On the 9'th day (July 18'th) I had rougher sex more accustomed to normal, after which my penis swelled up for the entire day. The next day I woke up in significant pain (a diffuse burning sensation around the entire shaft) and had substantial bruising. The pain and bruising lasted for several days. Every few days I experienced slightly less pain. I was feeling 90% better and got a blowie on the 24'th and only had a minor reaction the next day, so on that Sunday the 26'th I had very gentle sex with my wife. The next day on Monday I was back to significant pain again.

By Friday the 31'st I was feeling quite good, maybe 90-95% better. I decided I should try the fire roll massage because I haven't massaged since the 9'th day post op, due to my pain. Well after the massage I developed pain again, which got worse the next day again. I compounded that by having extremely careful gentle sex that Sunday the 2'nd (it was like 3 pumps lol...). Monday morning the 3'rd I was in quite significant discomfort again like I was after the initial flareup on the 20'th, even seemingly worse than before and like prior events, the pain has lasted multiple days and is ongoing. The pain again lasted for the entire week, and never fully subsided. For reference the pain is a diffuse burning throughout the entire shaft and is constant (sleep and heat is my only real relief) erections do not cause any pain. I have been abstaining from basically everything trying to recover, but I'm still in constant discomfort even today.

I messaged Dr. Rupeka's office on Aug 4th and explained the situation, and they advised this is consistent with what they've seen in some cases with Bio-stimulators in some guys who get above average inflammation reactions from the Bellafill. He advised to abstain from activities and manipulation until the pain subsides, then return to hot compress and massage of the firmer area. He did say I could add in an anti inflammatory, so i have been taking Aleve with Tylenol.
I messaged his office again this morning (8/12) giving him an update and am waiting for a reply. I also had a visit with my primary care physician today to rule out any physical concerns or potential infection or UTI (urinalysis sample taken-waiting on results).

Has anyone else had reactions like this where they have pain for multiple days post activities? When I palpate the entire penis, I'm not able to find any specific area that is sensitive or seemingly connected to the pain. I'm slightly worried this pain isn't going to go away. I’ve been unable to concentrate throughout the day and haven’t even been able to work at my desk. My wife seems to think this will likely go away with time. I'm hoping I can get some reassurance from people here on the forum.

I’m only 24 hours post op from my round 1 but I really think you need to let everything heal, you shouldn’t have even attempted sex for the first 4 weeks minimum. You’re constantly bruising that area and not letting it heal.

@Rejuvall I believe may be jumping the gun, it’s only 5 weeks post op, you have weeks and months left before everything is healed and the collagen process has completed. Talking about reconstructive penis surgery at this point is crazy. The pain I would assume is just from not being careful with the area while it heals. Let it heal, try anti inflammatories, the collagen process is still underway.

First, I am not a doctor, only speaking from experience here. As a first step, I would visit your doctor or local clinic and get some steroids like they use for allergies. These lower your bodies inflammation level. If you don’t want to tell them exactly what’s wrong you can see a telehealth provider and complain about a severe seasonal allergy attack. When I had my hybrid filler with Leonardo he gave a weeks worth of prednisone post op as part of the protocol, which seemed to help a great deal.

Either prednisone or a Medrol dose pack might help if it’s just your body overreacting to the PMMA. If you’re anywhere close to the border, you can get them OTC in Mexico at a reputable pharmacy. Give those a chance before doing anything drastic.

While Rejuvall was being a bit blunt, this is why a lot of us recommend a round of HA first to get the body used to foreign material. It also helps that the trauma of the initial hydro dissection (how they open the plane) isn’t occurring at the same time as the PMMA irritation. Above all, give your penis a rest. If nothing else, the initial encapsulation should be completed soon as it typically only lasts 2-4 months.

I sincerely appreciate the replies. I read the links you provided Rejuvall and thankfully I don’t seem to have any of the listed complications referenced on your website. I’ll take that as a good sign for now and will keep en eye out for any changes in my status.

As for the prednisone recommendation. I actually have about 50mg on hand. I messaged Dr. Rupeka to inquire if this is something I should consider.

My PCP today seemed to think this will heal with time. Same as what Mike is saying. My wife has done a great deal of research and seems to think the same thing. Apparently the Bellafill paperwork cites that several patients experienced dull burning sensations that resolved within 180 days. I’m keeping my hopes up for now.

I took just 7mg of prednisone yesterday and it seems to have helped quite a bit. What are the average dosages and lengths when people are using prednisone? I only have 40-50mg left, though I’m sure I can get more from my PCP.

Yesterday I messaged Dr. Rupeka asking about taking prednisone at this point. I’ll update when his office replies.

5mg twice a day is probably minimum dose. I once had a doctor prescribe 20mg twice a day for a severe allergy attack, but that was way too much for me. Only ended up taking 10mg twice a day for two days and then switched to 5. Of course I only weigh 165lb so bigger dudes will need more for the same effect.

Dr. Rupeka’s office messaged me back Thursday and prescribed a Medrol pack. I started with that Thursday evening and it’s been 40 hours since then. Some of the diffuse burning has subsided but now I’m realizing the pain may be coming from one or two firmer areas of the filler. It’s possible these have been the source of the pain all along. One larger firmer area of filler is on the top middle area of my shaft and isn’t visibly noticeable, but it’s significantly harder and has distinct ridges. Another is much smaller and under my shaft near the middle top area.

I’m discouraged the Medrol isn’t working. His office advised I should be continuing with the hot compress and gentle massage but it seems like that massage is causing the pain the flare up. The pain is still constant and I’ve been in a bad mental space with dark thoughts… I’m so worried this pain is going to be permanent as I’m getting little relief now. The first few flare ups seemed to resolve somewhat noticeably. Now it’s just constant and unrelenting now. I’m getting some periods of relief but it feels likes it’s constant. The first week of flare ups, Tylenol alone seemed to help, now I’m requiring Tylenol and two alleve to bring my pain down to a manageable level from a 4 to a 1.5/10. Even that is still quite distracting.

Does anyone have any more recommendations or suggestions on next steps? Should I wait longer to be evaluated by Rupeka or someone else? Can this potentially still resolve in time? Should I seek Kenalog or other treatments for these firmer areas? Keep massing them even though that produces long lasting significant pain?

Go get an MRI to see what’s really going on.

Also brother just take something to relax, at the end of the day whatever it is, it’s fixable somehow.

Could just be healing too, give it a few weeks, that Bellafill carrier oil takes longer to absorb no?

Are you taking the Medrol by the taper schedule: 6 day 1, 5 day 2, etc.. or are you taking a fixed dose a day. If your flare ups are too bad you may need a steroid shot and continued medication. I remember someone else having this problem for awhile, but I’m terrible with names.

Hey man if im reading this correctly. Everything was going great, from the new size, the pain level ect. Then you smashed the wife several times pretty hard core when feeling good. Now you have pretty persistent pain. So lets simplify this for a moment. You're Dick has been through some serious trauma and you partied hard with it soon after said trauma. Sounds like you over did it just like i did and many others on here when wielding our newfound manhood. Mine seriously hurt for roughly 3 months after I hurt it having hard sex soon after my first round when everything was going awesome. I remember thinking how I fucked it all up ect and actually had a hard time trying to conceal it from my wife while trying to have sex ect. Don't let it bring you down and dont jump to anything drastic. Chill out let shit settle in and allow everything adapt to whats happened and happening. It took a long time for my pain to subside and i can still remember the exact moment when i went to far and crossed the Rubicon. I immediately was like oh shit WTF happened what did i do. This hurts like a motherf'er. With these procedures and what our dicks go through its a wonder that it doesn't hurt more than it does. Just relax and try to take it easy now , especially when its feeling a bit better.

I think you might be talking about jick975. I’ve been watching Rupeka’s performance on this forum and his hit rate is pretty high in terms of patient satisfaction etc.

Without zooming in on the specific outcomes per user ; of the above only 4 expressed issues; and i know that two of those 4 were resolved, the other 2 never posted again. One of the two that had their issues resolved said it was kind of his (the patient’s) own fault to begin with. The vast remainder have been pleased with Rupeka’s work from what I’ve seen.

My recommendation to skepticalattempt would be to look at jick975’s (extended pain case) and all the above cases for yourself (just do a member search) to get a sense of what the end results have been like.

Keep us posted though. I’m also making a decision with the data I’ve come across. As you can see the recent names are on the list. (Committed2this. Engrmike and skepticalattempt)
(I offer no guarantee that this is every patient of his on the forum, just the ones I’ve spotted)

Thanks guys I can’t say how much I appreciate the support.

My concern is that from all the users I can find, none had persistent days long pain. I’ve been taking it super easy on the peen the last 2 weeks and haven’t been recovering like I did the first few times. The first time I was able to stop Tylenol within 5 days, before I flared it up again, then again similarly the second time. However since then the pain isn’t going away. Also unlike other users the pain isn’t just when I’m manipulating it, it’s persistent ongoing and not ending.

Additionally the first few weeks I had no pain with erections. I was able to get hard while giving the wife oral and it almost helped the pain subside. I hadn’t given her oral in two weeks or so due to my recent flare up, but she requested some last week and I had pain with my Erection. Also last night i was having a sex dream because I woke up in the middle of the night with a painful Erection, probably 5/10 on pain scale.

My wife wants to me schedule an appointment with a Urologist this week. I’m still continuing to not do any activities, and I’m going to cease massage completely as I believe doing the massage this Friday (per Rupeka’s office’s recommendation) may have caused it to flare again.

To be clear this all could definitely have been caused by the initial rougher sex we had on day 9, and I’m not trying to point fingers, I’m just trying to figure out how much hope I have and if I need to change my approach. I feel like such a dummy as I was doing so well; then decided to smash the wife while I had whiskey Dick on day 9. I remember specifically saying “wow this PMMA helps my Dick stay harder” when it wasn’t fully hard. I wonder if I physically injured it during that night? That could explain the constant pain? I’ve read stories of individuals who “almost” broke their dicks and it took several weeks for them to no longer be in persistent pain.

There is always value to checking with your Doc, let nothing said here interfere with that urge/instinct

Comparing jick’s case to yours I’ve noticed a few things:

1. You were 5.2 at widest and 4.8 at narrowest; jick was 5 even and 7.25 in length. If i recall you are shorter than that in length.
2. Jick had no drunken escapades mid-recovery lol
3. Jick took far less product than you did in round 1.
4. Jick had his procedure in January and wasn’t totally pain free until April.

Assessment: The guy who started bigger than you, still took less product than you and didn’t disrupt his recovery with wild sex jut a few days after ended up having to wait over a dozen weeks at least to be totally pain free under all conditions.
I know his pain was active not passive like yours but then again he didn’t mess with his recovery (based on his report). You have far more collagenesis happening in a tighter space after way more trauma. Talk to your doc by all means. But mentally I would embrace a longer timeline before you could be totally pain free. Read some of the other Rupeka cases in the forum to get a sense of what’s safely possible in situations like these.
