I'm on the list for surgery on granulomas at the left base of my penis. About 4 years ago, I tried fat transfer here in Europe. There wasn't much fat left after 6 months, and the remaining fat had accumulated on the left base. I've had 3 cycles of PMMA in the last two years, and the last one caused a serious infection. I have 69cc of PMMA, I'm significantly heavier, and I continue to have pain primarily on the left side. I'm considering having the fat granulomas removed. I think resistant bacteria have attached themselves during the infection, and I think they're compressing some nerves, which is causing me a lot of discomfort. I haven't yet figured out if the greater weight of the filler is causing this problem or if it's the granulomas. The procedure will be performed through the scrotum, and there shouldn't be any visible scars on the penis. I'm very scared of undergoing this surgery. A fat granuloma is attached to the PMMA, and I'm terrified of getting infected again. Unfortunately, I have no choice, as the pain in that area is debilitating. I'm hoping I can get better by trying surgical Removal.

To be frank, I don't recall ever coming across a fat granuloma in the penis, so you sharing this experience with us is both insightful & invaluable.

As a note to the Readership: I imagine this kind of immune response is rare since often times the fat injected is of your own, but after some reading it appears that it is possible that some fat cells die prematurely (this happened more often in the 2000's/early 2010's where Doctors would overfill, and it would be difficult, if not impossible for your body to integrate & vascularize the tissue adequately, which sometimes lead to the loss/Atrophy of the fat itself), and it was the byproduct(s) of this atrophic process that could trigger a granuloma -- in other words, fat harvested from your body, when injected correctly in low-to-modest volumes (which can be built upon in subsequent appointments) should present a very low granuloma risk, arguably lower than anything synthetic or manufactured in a lab (at least it would seem so in theory, and all my time following the scene, this may be the first case of a fat granuloma I can recall). Suffice it to say, please don't be alarmed if you've had fat injections in the penis or elsewhere and happen to be years out post-op. Just a PSA to calm any nerves since this kind of report is exceedingly rare.

Exceedingly rare and extremely important to share, that much for sure. We will watch this case with the utmost support. If you wish to share with us who you're going to or any other parts of the process, feel free to share.

Thanks for your reply; as always, you're very helpful with your information. The fat was transferred from my hips and stomach, approximately 20cc, and a second cycle of 22cc was performed three months later. In the first cycle, it was "enriched" with stem cells. According to the clinic I went to, this "new" technique should maintain its results over time; some would be reabsorbed and some would remain permanent. Unfortunately, with frequent sexual intercourse, the fat accumulated at the base of my penis. After six months, I no longer had any Girth except at the base of my left penis. Before my PMMA cycles, I asked Avanti Derma if the fat at the base could pose a problem for the procedure, but they reassured me that there would be no problem. After the first 2 cycles of PMMA everything was uniform, the third cycle with the infection changed everything, the soft fat moved even more towards the pubic area and became very hard forming about 5 "granulomas" which according to my Urologist could crush some nerves in that area causing me pain. Unfortunately, my history with enlargement has been complicated. I had a perfect penis after two cycles of PMMA, then the infection, the Circumcision, the pain I've had for a year, the skin completely loose, a color similar to chronic inflammation, now I'm on the list for surgical Removal of these granulomas and there's no guarantee the pain will improve, there could have been nerve damage due to the infection, I also requested a tissue biopsy to evaluate the presence of bacterial biofilm, although it's not easy to obtain consent through the hospital and I'll probably have to pay the costs privately.

I can sympathize about the pain, I had it for about a month after my first PMMA. It did go away fully. I couldn’t imagine having that pain for a year+ like you.
