Hello all, first time posting.
Went in for PMMA Round 1 at Avanti Derma on 7/30. I was impressed by Dr. Morales and Ian, they were really great and I especially want to thank Ian for answering all of my questions and being so good at patient care.

Overall Review:
So far I am happy with my results. I will likely go back for a second round in January or February for some corrections, but overall I would recommend the procedure.

I made a few mistakes:
- I should have had a better neck pillow and probably had the zen hangar in advance
- I should have taken the medical shuttle in the afternoon 48 hours+ after the procedure, spent the night in San Diego then flew out in the morning.
- I should have not used a Penimaster pro at day 7 to try to control edema. It caused some damage to my skin below the glans on the left side and shifted the filler maybe 1.5 mm. I can live with the results, but this was a mistake I attribute to a few things. One, I overcorrected because I took Creatine without thinking and my water retention went up for a few days and I felt I had to do something to control the edema, so I felt a stretcher pointing up would help. Two, the Penimaster Pro inflation ball is insanely hard to control and is frankly dangerous. Three, I didn't realize how fragile my skin would be post procedure. It's basically super warm 24/7 and tha makes it fragile to mechanical forces. The damage is a problem and somewhat permanent, but I can live with it, and I'm confident the minor issues it presents will likely be corrected with time and a second round.

I started making an hourly log of the whole process, but reading through it it would be a waste of time to post it.

I did make a condensed set of recommendations that I'll post as a reply to this post, that would probably be useful to others.

I started having sex again midweek into week 4. It feels different, but overall better. Both women I have slept with seem to be enjoying it, but they do get sore faster. I also only fit into larger condoms now. I used to be able to fit into the Okimoto Zero One (just barely, with a cockring effect that was nice but somewhat painful). Now I cannot practically fit, and I need to use larger sized condoms, my favorite now being the One Custom Fit, which I comfortably fit into a 64H, or a Magnum XL. Only the Mr Size and My Size Pro are slightly too large to wear, so I think a ~62-64mm condom fits well.

I may try to get back into extending so I can try to gain .25 inches in length before round 2.

The right side of my shaft looks great, almost nothing noticeable, the left size does have some asymmetry and perhaps nodules, and the left side is has a lot of veins so that changes the texture as well, and it has some flat spots that are not very noticeable. I'm confident with time it will improve. I also think that maybe 60-70 percent of the PMMA is mostly shifted forward on the shaft. It looks symmetrical for the most part, but I do think I did get a bit of a turtle in blanket effect when Flaccid. Luckily I think I have a relatively large glans so that definitely helps. Being thicker kind of makes my unit look shorter, which takes some getting used to.

I'll also mention I went back to the gym around day 12. I started with stairs, and by day 14-15 I was doing normal exercise again. Might have been early, I think the major concern here is infection risk which is more likely in the first two weeks.

Prior to Procedure
  • Two weeks prior
    • Start Collagen, Vitamin C, Zinc, Copper Supplementation
    • Start optimizing hydration
  • One week prior
    • Stop taking Cialis or similar drugs / herbs
    • Stop taking Creatine / anything that helps retain water
    • Avoid salt (this will reduce edema swelling + water retention)
  • Day of Procedure
    • Shave and clean the area thoroughly
    • Shower thoroughly

Post Procedure Recovery
  • Supplements
    • Plenty of Water
    • Cialis 5mg
    • Supplements (Collagen, Vit C, Copper, Lysine) continue to take until 6 weeks
    • Consume lots of animal products, high quality protein + bone broth
  • Stretching
    • Day 1-2 - Wear hangar as often as possible, except when sleeping. Continue to wear hangar as often as possible for 4-6 weeks.
    • Use only glans-based devices (e.g., noose-style hangers). Avoid shaft compression entirely the first 7 days.
  • Heating / Red Light Therapy
    • Use hot wet rags - Max 104 F.
    • Week 1 - Wrap around penis every 2–3 hours to soften filler.
    • Helps with molding and reduces risk of hard lumps or asymmetry.
    • Red Light therapy can be done 2x day for 10-15 minutes, but infrared should be used with great care and consideration, as it can increase edema which will make it difficult to determine where to massage. Red light does stimulate collagen growth, but this is an optional approach.
  • Massage and Rolling
    • Massage and rolling should be pretty aggressive in first 24 hours and first week. By day 1-2 most of the filler will stay where it is. Follow clinic instructions, always massage away from glans. Expect significant swelling so it may be difficult to be confident about how massage should be done correctly. This is why Creatine, salt, inflamation should be managed.
    • Avoid using OK/grip or massaging while Erect.
    • Do not massage with oil until cannula holes are closed
    • Frequency: Every 1-2 hours during waking hours, every 3 hours during sleep. Expect reduced sleep first few days.
    • Frequent erections are encouraged to stimulate blood flow and help detect asymmetries early.
    • Users suggest using porn to get Erect briefly, take photos, and mark areas for targeted rolling.
  • Sleep Positioning
    • Sleep face up, with pillows on either side of body to prevent turning in sleep.
    • Alternate between resting your shaft between legs and resting on abdomen facing up.
    • Avoid compression
  • Waking Positioning
    • Wear hangar provided as much as possible
    • Wear loose athletic pants, wear boxer briefs that are not too tight, slide hangar down leg and alternate. Avoid compression. When in room, wear loose pants only.
  • Bandaging and Leaks
    • Avoid showering until cannula holes are sealed.
    • Do not use lubricants until cannula holes are sealed and be careful afterward as the cannula holes in the second layer won't be sealed until day 10-11.
    • Use NEW medical gloves whenever handling the bandaging or touching anywhere near the holes.
    • Minor bleeding or oozing is normal. Don't try to seal it up prematurely.
    • If product leaks from entry point: clean the area and roll outward from center, not in.
  • Inflammation
    • Inflammation is normal, especially towards glans, it will likely reduce by end of week 2.
  • Sex / Masturbation / Devices
    • Avoid masturbation as long as possible the first week
    • Do not use pumps, non-noose stretchers (glans for example), or condoms or anything that grips the shaft until week 5 or 6
    • Avoid sex until week 5, week 4 at the absolute earliest. Some users have reported infections after sex at week 4 or 5. Use your best judgement. The clinic says to wait 6 weeks.
  • Psychology
    • Do not panic. Do not despair, you will be OK. Be diligent and take it one day at a time, it will get better. Many men report significant distress in the first month, remember to follow the process and be optimistic.

Weeks 5–6+
  • Resume full routines (stretchers, vac devices, gym, sex). Continue monitoring for nodules.
  • Expect to do a second round for any needed corrections. It is very common to do a second round, so plan accordingly.
