Hello everyone,

About five years ago, I underwent a PMMA injection. Aside from some minor nodules and granulomas, I’m generally satisfied with the results. However, I keep experiencing an issue where I develop a type of edema or fluid accumulation beneath the skin of my penis after sexual activity. I am Circumcised, and I'm curious if anyone has some experience or ideas how to handled it.

Also, can the fluid accumulation be treated with corticosteroid tablets or a compression bandage? Is there a way to expedite the resolution of this issue?

I was thinking of trying to get my PMMA removed, but unfortunately I‘m think the outcome would be disastrous.

Removal, especially under these circumstances, is both extreme and unnecessary. Removal should be a last resort stemming from much more serious complications.

It might make you feel better to know that it is not uncommon for men to report residual swelling for days after a prolonged sexual, pumping, or masturbation session with PMMA. While not described as an edema, I could see someone conflating the two. If your "swell up" isn't very red & painful and lasts only a few days, at present time I don't see it as anything serious -- although it would be good to reach out to your performing physician to be sure. Anti-inflammatory medication may help, but again, it must be green lit by your performing physician or a doctor who understands your situation.

Removal of PMMA is invasive and can open you up to a host of new potential complications -- this is why I say it has to be a last resort, where the pros of Removal outweigh the potential cons. For a situation like this (which I think is relatively benign compared to other potential complications associated with male Phalloplasty), I personally don't think it meets such threshold... not even close.

As a reminder, I'm not a Medical Professional, but more-or-less the Community's Patient Representative -- this implies that while I may be very well informed on these subject matters, I'm not an official authority in the same capacity a Doctor would be, and my comments are largely personal advice, arguments, and analysis.

Thank you for your reply. Since my last post I still have „fluid accommodation“. It became a bit better for almost a week, but as soon as I have days with several erections, fluid build up occurs. I have no pain, nothing is red or seems to be infected. What I noticed is, that since this longer lasting liquid build up occurs, I have 2 small hard knots. Possible that due to these new hard knots, more collagen is produced? Anyhow, I have a appointment at the specialist who injected 5 years back. I understand that Removal must be the last option. But honestly, if this newly liquid or whatever it is(new collagen?) , is now going to stay forever, I would feel totally uncomfortable(see attached pictures).Do you might have more information on Removal outcomes incl. common complications? But yeah, I hope Removal is not going to take place. Thanks & regards from Germany.

Ps. One more question: My understanding is, that due to the foreign material, the body starts producing new tissue around the filler. Does this process stop or is it a continuous process, even after 5 years?

It certainly is a strange type of swelling, akin to what I have experienced after any intense manipulation of the shaft (i.e. Penis Pump), but not to this degree, and usually not for more than 2-3 days.

Do you have any recent pics where the swelling is not there?

Also, by "fluid accumulation," are you just assuming this, or does it feel like an actual build up if some kind of bodily fluid or water?

Not sure if its the same issue as yours but the longer I have my Ellanse the squishier it gets. It feels as if fluid is “stuck”.

What you’re describing, I’ve also experienced in the past for a maximum of 4-5 days, for example when the condom was too tight. But what’s happening now has been present for two months. I’m planning to abstain from sex for a few weeks to see if this soft, swollen area subsides.

It feels very soft and doesn’t seem like tissue, so I think it might be fluid. What also catches my attention is that it only started happening after I noticed two nodules. At least, the timing seems to match. Unfortunately, I don’t have any other pictures.

I have also had this experience. The first 2-3 years, everything was firm and solid. But now, it’s very spongy, especially over the past few weeks. My theory is that the body is continuously producing new collagen or that increased fluid accumulations are forming due to foreign body reactions.

Update:
Today I learned, that the injected material is technically not PMMA. It is correctly called polyacrylamide, an aquafiller brand named Losdeline, which has been taken from EU market 2 years ago.
Still hope to find some answers on how to get rid of my symptoms.

Ellanse is supposed to go away though? So I hope that happens then I would probably go with proud Urology for a megaderm. I can almost feel a “puddle” of water in the middle of my shaft. They will reccomend you just get more fillers but Im not sure if that is a good idea.

Jap! Wondering why it has been allowed to inject here in Germany until 2023 while it is even bannend in China since more than a decade.
Have an appointment tomorrow to discuss an way forward… we‘ll see
