Hi Dr Oates

Since you do most injections Flaccid know, I would like to know what determines whether you inject Flaccid or Erect? Is it the size of the penis in the Flaccid state?

I think you at some point wrote that it is much easier to distribute the filer when you do the injection Erect.

I also have a related question.

Someone also said that for 2nd round Ellanse, that you needed to use needles rather than a cannula. If that is correct, can you explain why exactly that is the case for Ellanse? Does it have something to do with the inflammation response?

Also interested to know about multiple rounds of Ellanse. I\'m sure Dr Oates at some point wrote that he wasn\'t even sure that it would be possible.

I talked to Moorgate Andrology and they said that it is possible to top up Ellanse, but that I would have to wait a minimum of three months before topping up.

Yeah, Morganstern told me minimum 4 weeks, usually recommended 6 weeks or more. It\'s a matter of how soon you get over the initial inflammatory process.

Dr Oates, I have one more question.How is the procedure performed if the patient is uncircumcised. Are the injections made with the foreskin skin pulled back or is it kept over the head?

I ask because if you look at this picture of one of your patients who is circumcised, you have been able to make an injection hole past the circumcision line and very close to the head. Is that an advantage that you don\'t have with uncircumcised patients?

Hi everyone,

In guys with lots of shrinkage (think exposing yourself in a medical setting, a guy you just met approaching your penis with a big needle, it is already shrinking because of the numbing cream...) then it can be easier to induce an Erection. We have developed techniques to make it easier to do Flaccid over time. On some second sessions we will induce Erection if they tell us it looks fine Flaccid but there is a low spot when Erect.

For Circumcised guys doing Flaccid then yes we tension the Foreskin over the glans. For cut guys sometimes I will use a point past the circumstances scar, especially if it is difficult to get all the way down there with a cannula from below. Sometimes it can be a bit stuck down there. It is probably easier to go past the scar with a needle - but I prefer to use cannula.

As to when to do topup with Ellanse. It is pretty easy in the first month because you have not formed much collagen yet. Once all the collagen if formed it is a little more difficult for the cannula but easy with a needle. But it is not impossible with a cannula.

Have you experienced cases where a mild case of Phimosis in a patient had a negative effect on the result of the procedure or on the aesthetics? I guess that if you have a tight phimotic band this will only get more pronounced with an enlargement. This could cause problems, especially when pulling the skin back making the phimotic band squeeze the upper penile shaft.

Also, how is it progressing with training doctors in the UK?
