Just updating this thread - what would you focus on here? I tried to ask some guys online on reddit, but I think they were being too nice...except a few who may have been more honest...not sure.
I have to go back to get the biopsy wound closed, so I can ask my uro to do a little more to complete this once and for all.
1. Complete the circumferential incision so it\'s not just halfway around
- My derm thinks this is risky to do now - maybe see how it heals and wait for December... I might want the extra skin in the long run.
- I have a Nodule in that section, though..and it\'s painful sometimes. So, it could go.
2. Small dog ear Revision (lump on top)
- Uro agreed to take this out as much as he can. Doesn\'t see an issue with it.
3. Lumps on the left. These have been injected and partially excised. They don\'t feel weird because they\'re soft...so i never bothered with the aesthetics of them. One could come out if I did the circumferential excision, but then i still have one toward the base.
4. Will use scar gels (celacyn) and possibly silicone too...for the next 8-12 weeks. Once I get the go ahead, I\'ll use a stretcher & start with periodic viagra doses to help things expand again.
This was my Flaccid penis before R3...right before I went in to get PMMA.
This Nodule left Nodule and right side cutout makes it look really weird to me, and I\'m not sure I\'d be comfortable walking around any shower or locker room like this.
The right pic is sort of how I think it would come out if the surgeon took the same amount off the left.
With only one side being cut, I have a lot of extra skin...which also looks / feels kind of weird.
Financially - it\'s free if I do it this year due to my insurance. If I do it after 1/2016, it will be $2500 at a minimum...and after everything, that\'s not a small amount.
So, how bad does this look? What do you think? I wouldn\'t be comfortable naked in the Flaccid state around other guys with how it looks now...which was part of the point of spending all this time and money on PMMA.
One other pic of the current state:
Erect pics are below the next two posts by hunkydory and Bigben.
I never asked you if you tried Biafine on it. That technique is actually something I learned about from you, as you\'ll recall, and while my glans scars are still there, I have to say that Biafinehelped tremendously to get them at least sealed up to where they aren\'t leaking everywhere. It sped up the healing and within days I could see new skin growing over the area. At this point the Biafine has stopped making a noticeable difference, but it definitely helped in at least getting things primarily healed aka not bleeding or leaking and at least some form of shitty skin to grow over the area and seal it.
So, I used biafine constantly on my last wound... and it didn\'t heal. So, I kinda got the impression that it wouldn\'t help this kind of wound - though it helped you a lot...and helped me in the past. Also, it is very \"wet\" so to an extent, it macerated the peri-wound area which isn\'t good and could lead to wound expansion. I started using medihoney (don\'t recommend that either) and bacitracin... but those didn\'t do much and I didn\'t have any infection anyway.
Finally, my doc gave me Skintegrity and it seems to be doing an ok job keeping it clean / moist without macerating it (though it still does, a bit). Also, celacyn will debride the dead tissue, but it\'s so complicated with the foreign body in there.
Kenalog and 5fu, however, seemed to be a different story in a few doctors\' opinions, one even sayiing that he has seen \'miracles\' from using it on PMMA nodules that formed in the face. Also, all of these doctors said its not uncommon to need multiple treatments spaced weeks apart (usually 5-6 weeks) in order to see a difference.
Yeah, I read that comment by Arnie Klein (Michael Jackson\'s Dermatologist). It\'s sort of uncharted territory, but I tried 2-3 injections per site over 2 months, roughly. If it did something, I would\'ve kept going..but there was nothing really happening. It helped break up the band under the shaft - but then those turned into separate lumps where I guess the Kenalog \"cut\" the fibrous band. The docs I saw wouldn\'t go to Kenalog-40 (citing major Atrophy risk) and Dr. C said Kenalog-10 won\'t work on PMMA nodules, so I was kind of out of options.
Here\'s a small piece of my injection schedule. I kinda think my nodules were just a surgical problem...or at least, the docs I saw said it was. If there was more PMMA experienced docs out here, I\'d know. The derm who removed my lump injects artefill all the time and sees this in women who keep getting layer after layer. He tries not to make excisions from the face unless absolutely necessary...but he and his partner try to break it up in various ways (saline early on with massage, Kenalog, and then 5-fu).
Thanks Smalljay. Those are really good points. I think I am going to leave the left side alone... even though it is uneven and there\'s a lump. Removing more skin is just asking for problems like you said (erections too tight, and it would draw up more testicular skin to the shaft, etc...)
The biopsy wound never closed. It was an excision of the PMMA in my dermis that was actually causing pain at the time. So, he has to cut that out and re-close it again. I figured he could do more, but it is probably not worth it and I shouldn\'t let the insurance issue in the equation. Maybe in 6 months, I can really reevaluate this - and it could all be decent looking...and I\'ll be happy I never took the skin off. Once it\'s gone, I can\'t get it back.
The Nodule on the left is kinda hard in the Flaccid state but soft and vein-like in the Erect state. It was trimmed by my derm and injected with Kenalog / 5-FU. So, it\'s a little better.
The problems I had with LS showed up right away as unusual swelling...which probably allowed the LS to seep into those areas. Or, it just was in those areas and caused swelling... then around 10 days things started to get really firm, especially under the shaft where I got Kenalog right away (but the nodules didn\'t go away 100%). The issues were obvious by 3 weeks when the swelling was gone. I may be off on dates slightly, but my journal has pics in sequence that is a little better.
I was saying I really wanted to look like that top Flaccid pic, which was taken minutes before I went in for R3 in the grand hotel.
The surgery I have to go through next is to close the punch excision (which should have closed on it\'s own but the tissue is all fucked up under there).
He only does surgery in the O.R. I am going to have the IV, blood pressure cuff, O2 monitor, etc... and this is just to do a wound closure for 20 min. So, it is the \"right\" place to do a large circ excision (not doing that in a derm\'s office....). Once I am going through the whole hassle of the surgery, and knowing I am uncomfortable with the appearance of the left side (and there\'s a Nodule in there anyway) it\'s a good opportunity (and probably my last opportunity) to ask him to fix the left side...taking out the Nodule and the excess skin. He may decline to do it himself due to risk/reward.
I just really think I\'d be self conscious walking around with that weird 1/2 bulging Dick...basically equivalent to my old 3\" Flaccid thin penis.

You look great - leave it alone! You have already had more surgeries on your Dick than 99.99% of people. Knowing when to quit is sometimes a hard decision to make when there is always a voice telling you I could just look like this, if I just did that..
Thanks guys. The left side has a lump of PMMA in there that is causing the bulge in addition to the extra skin (about 1\" Flaccid was removed). It just (to me) looks so much worse now in the Flaccid state. I want to get back to where I was in the older pic...before everything went crazy. I\'m close. Losing surface skin doesn\'t affect Erect length I don\'t think...it might make the Erection very tight for a few months. I wonder what happens to men with skin cancer on the penis. They must have these issues. Here\'s some Erect pics at about 80-90%...but these are from 2 weeks ago so the scar is smoother now. I just don\'t have new pics. That punch excision ... still is open.. (Here\'s a \"before\" pic too - after 2 rounds of metacrill).
This was all done to resolve this (which was worse in person... lumps were much deeper and harder than pic can show):
I also think if it looks ok Erect I would just leave it. I don\'t think the Flaccid is bad at all and it will get better over time as the skin stretches. And of course as HD mentioned you can help that process along.
If you get another surgery you are going to have to stretch anyhow to get length back, but as it is you only have to stretch one side to get symmetry. This should be easy and it looks excellent given how recent you had the Nodule removed.

Restoration, is the angling just from the skin being tighter on one side? If that is all it is, I would say don\'t have anymore surgery. Skin is so elastic I would just hang and stretch it out. Just hang on the opposing side and you will feel more on the side with the tightened skin- it will surly stretch. You have nothing to lose and if I were you I would avoid anymore surgery. The fact that it looks better Erect means the skin just needs to be loosened. I don\'t think it looks bad at all - when you consider what you have been through it could have looked really bad. I think you look great - rather than remove skin I would add through stretching. Just my 2$.
