There’s a growing trend I've noticed among enhancement providers to market their “deep injection” technique as the reason their Girth procedures look more natural or last longer. But from a surgical and anatomical standpoint, injection depth has little to do with aesthetic quality — and everything to do with safety.

At Rejuvall, we frequently see men seeking Revision and corrective treatment after being injected too deeply or in the wrong tissue plane. These cases don’t just involve visible issues like nodules, granulomas, and migration; they sometimes involve vascular injury, loss of sensation, painful erections, and erectile dysfunction caused by misplaced filler compressing or disrupting the underlying neurovascular structures.

Understanding the Layers That Matter
To understand why “deeper” is not necessarily better, it helps to know what lies beneath the surface:

1. Skin and Dartos fascia (Superficial Layer)
This is the safest plane for Girth enhancement injections. It provides a defined compartment for the filler to integrate naturally with the tissue.

2. Buck’s Fascia (Protective Layer)
This tough, fibrous sheath encases the erectile bodies and protects the deeper structures. It acts like a natural barrier, and filler should not be placed beneath it.

This dense, inelastic layer wraps the erectile chambers themselves. There is no natural space between Buck’s fascia and the tunica albuginea where filler could safely or effectively sit. Attempting to inject in this plane simply forces material into tissue that wasn’t designed to hold it.

Depth Doesn’t Create Aesthetics; Skill Does
The appearance of a smooth, natural enhancement depends on precise layering, material selection, and the injector’s understanding of penile anatomy, not on how deep the needle goes.
