If you could do your first round of PMMA again:
- Would you "go big," or would you stay conservative with volume for the first round?
- Would you go to a practitioner who uses a blunt tip cannula or to one who injects with a needle?

I've skimmed through most progress reports on both 3.0 and 2.0, and the more I read, the more confused I am. Most guys going to Avanti seem to be getting like 16 to 24 cc of Linnea. Another popular practitioner doesn't hesitate to give 35 syringes of Bellafill in a single session.

On the other hand, other practitioners say that 15 syringes of Bellafill at once is a huge amount, and they recommend like 8 to 10 for a first session.

For guys who already have PMMA, if you were doing your own first round again, how much PMMA would you start with?

And do you prefer injections with a blunt tip cannula, or with a needle? Or is it all the same in good hands?

Looking to do my first round soon, so I appreciate the feedback! (Also happy to hear from practitioners.)

Much of this depends on you and your goals. If you're longer, 20-30 cc might be appropriate initially, while for shorter lengths, starting more conservatively could be the way to go. Splitting 30 cc into two sessions provides an opportunity for fine-tuning, but I understand that multiple visits might not be practical due to budget or time constraints. Additionally, spacing out the injections slightly reduces your risk of granuloma formation.

Regarding injection technique, both needles and blunt-tip cannulas can yield excellent results in skilled hands, but I generally prefer needles for greater precision and control. Dr. S

Thanks, @Dr_Sullivan . I really appreciate your input--great information on your website, too. From your girth calculator, I would be looking at maybe 40 to 50 syringes, so I'm sure there would be multiple sessions with any provider (the question is how many sessions and how many syringes at each session).

Is there any risk associated with needles due to piercing the skin and fascia so many times (relative to the number of skin breakage needed for a cannula)? Especially with a lot of superficial blood vessels that have a risk of being pierced? Or is this not really an issue in your experience?

A practical plan is about 15 syringes per session (so three sessions); depending on your anatomy this could run a few syringes lower or higher each visit. Although needles create more entry points than a cannula, the risk remains very low. We routinely perform microneedling and laser procedures that pierce the dermis thousands of times, and that controlled micro-trauma actually stimulates collagen without complication.
