Correct me if I'm wrong, but we don't currently know if the reason for Ellanse's longer than advertised duration is that the collagen is sticking around after the spheres have absorbed or if the spheres themselves are taking longer to absorb than anticipated. Maybe you could tell with some sort of scan?

Well PMMA is guaranteed permanence, whereas Ellanse's duration is a guess, in that it could eventually dissipate, but not as early as the manufacturer suggested (perhaps maybe why it's having issues with FDA Approval in the U.S., but that's just speculation on my part).

That said, Ellanse's longevity thus far makes it an attractive filler in that the microspheres themselves do in-fact dissipate, it's the type of collagen produced (similar, if not identical) by PMMA, and that collagen's half-life is seemingly hard to get a precise answer on (I've tried researching online but I may have to defer to the Doctors here in hopes of a clear answer). PMMA ensures that there is always a neocollagenesis and angiogenesis process by virtue of simply being there (too large to break down or remove by the body, and too small & biological inert to raise red flags by the body, hence its effectiveness in cosmetic medicine). Ellanse's eventual dissipation of its PCL microspheres may put a lifetime on the nature of the collagen type produced, and how long that lasts is seemingly something I'm currently investigating.

There is no such thing as the absolute best, many of the best Doctors are innovators in an otherwise experimental field. If you search the Physician Directory I linked above, you'll find that they are very capable in their methodologies. What you need to assess is logistics at this point if "money isn't an issue." PMMA is the the safest permanent non-surgical option, Alloderm (Surgimend is under recall at the moment) is the safest surgical method (under an experienced Surgeon), and the P-Long protocol is the safest means that don't involve injections (other than monthly PRP) or scalpels. Permanency in it of itself isn't necessarily superior to temporary fillers like Hyaluronic Acid (HA) as it is more recognizable to the body than foreign objects like PMMA, Ellanse, or Radiesse micro-spheres. Food for thought, but they are all viable options as iterated in my prior post.

Honestly 3 inches flaccid or erect is impressive. For growers like me, a 3 inch flaccid increase would make me look like I have the potential to be "hung" even though I'm very mediocre in the length department (girth carries me).

Viable options still in discussion, in a nutshell:


Non-Surgical (Injectable Dermal Fillers):
  • Hyaluronic Acid (HA)
  • PMMA
  • Ellanse (not available in the U.S.)
  • Radiesse
  • Renuva


Surgical:
  • Fat Injections (FFT)
  • Dermal Grafts (e.g. Alloderm, Surgimend)
  • Ligamentolysis (lengthening surgery)


Protocols (neither surgery or filler injections; primarily relies on a prescribed enlargement routine and possible supplementation over a period of time:
  • P-Long
  • A soon-to-be-announced Rejuvall Protocol


Also worthy of note* -- rigid silicone implants for the purposes of augmentation are still just as bad as they were in 2011, and large volumes of silicone oil as dermal filler injections are arguably just as bad -- avoid both!

Yes , and I had ellanse. I don’t believe that the lig cut had any effect . I gained 3 inches of flaccid length — that’s it .
I started pe about 8 months after my surgery ( which included fat pad ) and got a small amount of gain in 6 months of work before moving to p-long . Extending does work but p-long accelerates gains.

Lots of options now ranging from fully temporary like HA filler to short term stimulatory fillers like Radiesse to longer term stimulatory like Ellanse or still permanent stimulatory with PMMA.

Currently at 5 years 3 months of the 4 year version, have not lost anything. If I do, I'd definitely do it again.

Metacrill was the PMMA Brand that was distributed in Europe, but if memory serves me correctly, it is no longer being manufactured. This actually might imply that there is no medical-grade PMMA dermal filler in Europe at present time (I cannot confirm 100% as this is being typed).

This would make Ellanse the superior option. For those who don't wish to fly to the U.K., Dr. Horn has a Brussels location. Ellanse has been shown to last much longer than originally projected (2-4 years) and is very similar to PMMA in the way it builds girth (through collagen creation).

My understanding from past forum research is no one has lost their Ellanse size yet.

I’m year 1.5 on a 2 year product and am still same size.

Anyone lost size yet with older Ellanse?

I’d plan on stopping PE after Ellanse. I was a little too aggressive after Ellanse with PE and had a noticeable decrease in symmetry as a result as I healed. Could have been from something else, but based on my experience I’d plan to do at least 6 months of no PE aside from a very very Light hang after Ellanse or any other filler. Also, once you are super thick may not care about much more length
