I would be interested to know what patients are thinking about in regard to the HA and Ellanse procedures.

I agree some of our updates are from years ago now, geological timeframes in penis enlargement as one member put it.

I will share a couple of things I have come to conclusions on over the past couple of years. I am not a doctor, so these are conclusions based on patient results and feedback rather than medical science. Regardless I think some of the conclusions can help guide people.

Too much HA is being injected per patient.
We have stopped this at Androfill as far as I am aware and have had warnings on the website about injecting too much filler for over 2 years now.

However, almost every other clinic and doctor doing HA (even the good doctors) still inject far too much filler. There is no other way to explain it aside from greed overriding patient care, assuming the doctors in question know that injecting too much is an issue.

Why is it bad to inject too much HA? (this means over 30ml, but probably over 20ml in many cases).

1. Stretching the tissue
The tissue in the penis is stretched irreversibly, including the skin. If too much HA is injected it means that a reversible procedure has led to an irreversible outcome.
Put simply, if you increase your girth from let's say 4.5 inches to 6.5 inches, and then decide you don't want HA anymore, you will be left with loose skin.
Picture an obese person that loses a lot of weight. Injecting too much turns a reversible temporary procedure into a permanent change.
If you increase your girth by 2 inches with HA, you can not go back to your original penis.

2. Natural feeling / slushiness
HA wasn't designed to support such a large increase in size. With too much HA the penis will feel very soft, particularly if you are not fast at metabolising it and so are left with filler in the late stages of isovolumetric degradation a year or so later. A penis with 40,50,60ml of HA is likely to feel slushy after 18 months.
If you have just 10 or 20 ml you are more likely to metabolise it all before it turns to slush.

Putting to one side the isovolumetric degradation issue, when the filler is freshly injected even fillers with the most structure (Volux, Teosyal UD) cannot support 2 inches in girth increase. The maximum increase with HA should be 1 inch for this reason alone (to feel normal), and also so as not to stretch the skin (point 1. above).

3. Uneven reabsorption. This is made worse when the layer of filler is thick.

Cheap filler
The use of cheap, counterfeit / made in China, parallel imported filler, or even completely contraindicated filler (Genefill / Hyacorp) is common in the UK.

I am not sure how a doctor can read a warning pamphlet inside a box of Genefill which clearly says "contraindicated for use in the genitals" and still proceed to inject it.
There is a case we are dealing with today in which a well-known doctor on Harley St, with a big Andrology practice, told the patient he was injecting Voluma, showed the patient the box of Voluma (for the first syringes) and then proceeded to inject some other type of filler for the remaining 20ml (later discovered to be Genefill upon investigation).

If patients do not get better at asking for the stickers from the box of filler, and observing and counting every syringe that goes in, they will continue to be a) ripped off, and b) injected with who knows what.

Men seem much less likely than women to check on what is being injected.
I have come to form a view from talking to patients and support staff at other clinics that around 70% of the product being injected is not what the patient thinks they are getting.

It annoys me because for a few reasons. Aside from the ethics of patient consent (if you agree to have one thing injected and it is another product, then you have not consented), and aside from commercial fraud, it also makes the clinics using authentic filler seem more expensive.

The only way to compete with another clinic advertising 40ml of HA for £2,000 is to also use a lower grade filler.

For a 40 ml procedure using Genefill filler, it will only cost us £200 in raw filler.
For a 40 ml procedure using Volux it will cost more than £4,000 for the raw filler alone.

I have been tempted to name the doctors and clinics in the UK doing this.

Ellanse
Long term followers will remember how hesitant we were at first to inject Ellanse.
It took years before we were routinely offering it and usually only to repeat patients who had an idea of what they were getting into.

I still feel it is very risky for first-time patients.

1. Consider, what if some aspect of the patient's penile anatomy is not suitable for filler generally (for example loose pockets into which filler wants to migrate), or tight bands of skin that pinch in and cause an uneven shape. Patients are better to discover that are not suited for penis fillers with a reversible substance like HA.

Otherwise, if they are given Ellanse and discover they are anatomically a poor candidate, they can do nothing to fix the situation for years if the shape is wrong, the filler has migrated to the underside or any other issues.

2. Overall, despite Ellanse being a higher risk product (mainly the fact it can't be reversed), the results are generally better than with Hyaluronic acid, and will unlike HA support a larger increase in girth, to maybe +1.5 inch, without compromising on hardness.

My ideal pathway into penis enlargement with dermal filler would be:

1. To begin with a small 6-10ml HA procedure (to practice moulding and to make sure the filler sits well in the penis / the penis is anatomically well suited to filler without the higher likelihood of issues such as migration).
2. Then 10-20 ml of Ellanse (over 2 or more visits) to bring the maximum gain to something like 1 - 1.5 inches.

One issue to consider with Ellanse. It can become difficult to inject more using a cannula after around 3,4 months. The cannula simply can't break through the new tissue stimulated by the Ellanse / PCL.

I am not sure which other information is useful to patients. Perhaps what I am sharing here is already known.

What other sites exactly? You mean the Penuma subreddit that does some soft censorship (I posted once or twice asking questions about the complaints people have made on places like this forum and they weren't having it). I say "soft" because they do still let a good number of dissatisfied and/or removal patients post, although if they still exist I don't know (??).

As someone who's moderated the only forum devoted to the topic of surgical penis enlargement for over a decade, I would strongly suggest taking my word that fat transfers aren't as "dangerous" as a rigid silicone implant, in-fact, the fat is your own tissue. That's neither here or there.

As for non-surgical girth options, there are a few popular & viable ones spoken of today:

Hyaluronic Acid (HA): Temporary filler that provides instant volume. Temporary implies that you may require top-offs in the future as the product will absorb over time. Anecdotal evidence suggests HA can survive 18+ months before a top-off. HA naturally occurs in the body and is relatively safe for cosmetic use.

Ellanse: Long-term temporary filler that stimulates collagen growth for girth. This is predicted to survive 2-4 years (possibly longer?) before any necessary top-offs.

Radiesse: Much like Ellanse in how it generates new girth, but not reported to last nearly as long.

PMMA: The only recommended permanent filler option available. Trade-off is travel to Mexico (Avanti Derma), since the American iteration of PMMA (patent-enforced) is not only very expensive per volume, but uses a bovine carrier making it less ideal for penile application.

Avoid silicone fillers.

The aforementioned, especially HA, Ellanse, and PMMA, are most popular here. Deciding what's best for an individual depends on a number of variables (advantages of temporary-vs-permanence, travel & costs, expertise of practitioner, etc.)

I agree with Buckeye about being completely transparent with your healthcare provider, it's in everyone's best interest to come clean so that a proper diagnosis can be made.

A urologist would be the person to see, yes, but I'd also recommend seeing someone who has actually performed corrections on men with filler injections, including silicone... at the very least for a second opinion in the event your first doctor recommends anything drastic. Dr. Carney comes to mind, he works out of Atlanta and I'm fairly confident he's dealt with silicone filler issues before.

Either way, silicone in it of itself isn't a "sentencing" for your penis, and there are people with silicone that are okay, but this is a stark reminder to newcomers how essential it is to find ethical and experienced practitioners using either hyaluronic acid (HA) or dermal fillers like Radiesse, Ellanse, or PMMA, (no silicone) and that even going down this route isn't without risk.

I think you'll be okay, and if you have the time/budget, Dr. Carney is an option worth considering because you won't have to feel the need to "hide" any details and you'd be talking to someone who has dealt with your specific issue before. You have to remember that modern phalloplasty is still relatively "new" and many healthcare providers might not have a clue what you did or how to go about treating complications, hence my recommendations. Well here is the medical opinion -
Tadalafil (Cialis) the long acting version of Viagra makes a difference to many guys. There is a low dose, 5mg , that can be taken daily. they use is following prostate surgery to prevent long term shrinkage becoming permanent (there is something on this in my next Podcast/YouTube show with a penile rehabilitation nurse).
Alpha Blocker medication like Prazosin/Minipress. Decreases blood pressure by opening up blood vessels. Dont combine with the above/Viagra as the decrease in blood pressure may lead to death, according to the information sheet.
Yohimbi - supposedly has mild alpha blocker effect and maybe that explains its (unproven) reputation for increasing penis size. If it is an over the counter supplement where you live give it a go. Not available in Australia at all.
Testosterone level - dont know anything here on shrinkage so have to go by the stories you get here.
Ligament release - may get it to hang out more but think the reasons to get retraction are all still there so not going to stop it.
Stretching/hangin etc - sorry just dont know enough here.
Fillers - HA/PMMA/Ellanse - do make the penis heavier. Do provide some physical resistance to retractions. May make you feel confident in people seeing what you have. So yes lots of guys say they get LESS shrinkage after fillers. But for super shrinkers, guys who end up with an 'innie" rater than an "outie" it just won't work.

It seems as if you don't have an accurate perception of how you truly rank against other dudes and what women would think about your cock. I can almost guarantee that most women will find you large as you are ... and many will tell you. While this shouldn't deter / encourage you as far as GE goes -- all dudes want a bigger dick -- it may give you a little peace of mind. Your ex sounds like a bona fide size queen -- which is cool -- but her judgement probably shouldn't be taken to heart (and it seems like it may have messed with your head). Go out to a sex / swinger's club for a night and have a look around ... it would probably surprise you.

Also, you seem worried about your flaccid, and while, yes, while we'd all love a big floppy dong to show off in the lockerroom, this isn't the penile state that matters. Although fillers will significantly add to flaccid length ... at least in my experience.

Anyway, I say go out there and rack up some more experience and only then take a look at yourself.

I wouldn't trust a doctor who would perform a procedure on you with the fillers you bought on the open (or black) market. If he doesn't have a licensed distributor, it's highly unethical to take walk-ins with people handing them syringes in my opinion... nevermind the risk of just a bad result.

Also, I totally believe in competition and bring options to my readers and better prices; however, this isn't a field like female cosmetic medicine, where every other clinic on the block offers a procedure. Penile augmentation remains an experimental field and there is indeed a skillset in providing both the know-how and satisfactory results, and they come at a premium. In other words, if I wanted a tattoo, I want a reasonable market price, but I'd certainly pay a "little more" if that meant getting the best tattoo artist in my town versus a guy who just opened a shop. That "premium" gives me added reassurance I'm working with an expert and not just someone taking advantage of a cash cow operation.

My hope is to continue bringing on Sponsors to drive a healthy competition. SIZE MATTERS?

They say size doesn't matter, but it honestly depends on who you're with. The G-Spot is easy to find, but some girls simply want to be "filled".

Any Vac Stretcher will likely do, if it's comfortable. I intend to use a Vac Stretcher, so that I can combine it with a heat pack on the shaft (I'm still researching this though) and in the future, to keep the penis stretched following Hyaluronic acid or Ellanse on the shaft.
I purchased the Phallosan Forte (not the Phallosan Plus), because I heard the Plus version is NOT as good as some other brands. The Forte is meant to be good though.

Before exercising, we should stretch to "warm up". I realise the penis isn't a muscle, but perhaps this can "warm up" the ligaments, loosen the tissues and help the blood flow better ...thereby increasing the efficiency and safety of a stretcher. I found these pads that last up to 10 hours, perfect for an all day stretching regime. This is why I want to use a Vac Stretcher, so that I can combine it with a heat-pack.
  • I haven't tried the ones listed on Amazon yet. Has anyone tried long lasting heat-pads or similar? What were your results?
  • Were there any negative side effects?

I actually don't believe that the results are permanent, just semi-permanent.
I view this as a solution to the flaccid state. If I get erect girth and length gains, great! No expectations = no disappointments.
  • Is it best to use the Bathmate in the morning. Then wear the VacStretcher after to take advantage of the Bathmate's gains?
  • Or is it best to use the Bathmate in the evening to maintain the VacStretchers gains while sleeping until the morning?

LASER HAIR REMOVAL
They say being shaved helps with the appearance of a larger size. Personally I prefer the feel too. I hate shaving down there, it takes ages and it's a hassle so I decided to get laser hair removal down below.
I heard some people take viagra to make it easier to remove the hair on the shaft, but I'd rather not. This where the BathMate comes in! Goal: I think that by using the combination above and finishing off with HA, I can easily get E.Girth 6 inches.

What I have read about radiesse is it is made up of a naturally occurring Ca hydroxyapatite sphere. It works very similar to the other permanent and semi permanent fillers by stimulating collagen production.

Radiesse is temporary, and it only last 1-1.5 yrs. the problem is it is known to form bumps, especially in thin skin. Also, a big negative is it’s consistency. It is very thick. This becomes a problem when placing it. Very difficult to uniformly place it for esthetic purposes. Also, because of its thickness, when giving injections, one can not aspirate or pull back on syringe to verify one is not in a vessel. U do not want any filler in a vessel. Therefore, for aesthetic reasons, inability to dissolve, and higher likelihood of vessel occlusion, radiesse is not a good choice for penile filler.

Polyactic Acid (the primary ingredient in "Powerfill") is a bioplastic obtained from lactic acid according to my initial web searches.

There is a world of experimental fillers in regions that are too distant linguistically for adequate translations (Korea, China, Russia, etc) to keep up with, and quite frankly, I've never heard anything positive from what few reports I've seen (and I'm including progress reports as well as physician's opinions).

That doesn't mean we shouldn't learn about them, heck PMMA for penile use was being used in foreign (non-English speaking like Brazil & Mexico) countries before gaining popularity with American & European men. - that said, due to the considerably larger linguistic & geographical gap involved in examining these alternatives, much greater scrutiny is needed.

If anyone does have experience with this filler do chime in, and any Biochemist or Medical professional familiar with said-filler should also chime in. Personally, if there is no apparent advantage with this filler and its only promise is a temporary 2-3 years, why not instead just have Ellanse? It's already approved for us in Europe and Mexico, and is in trials with the FDA with eventual approval in the U.S. I mean at least until much more is known about said-filler. My 2 cents.

When girth is added to the penis via any sort of injectable or FFT, that enhancement occurs by adding the bulking agent into to the subcutaneous tissue “surrounding” the vascularized penile organ. The incredibly small capillaries located within the subcutaneous tissue are not affected by total body blood pressure – they are simply too minuscule and remote to be impacted. If you added additional girth augmentation later, our limitation is the degree of stretch within that subcutaneous tissue – not the penis organ. Human tissue is very stretchable with time. You just can’t force stretch it without risks.

Your erection process to attain a full engorgement is an automated sealing process within the organ. If you provided increased blood force it won’t change the amount of blood provided to the penis as it locks itself closed once a full dose of blood is in place for optimal penetration size. Alas, high blood pressure tends to cause an opposite effect on men and their erections as veins in the body can and will harden from high blood pressure, resulting in erectile dysfunction symptoms from insufficient arterial blood flow. Remember, your penis is located on the very edge of your body. That means it gets lower priority of blood flow when resources are limited – as with patients exhibiting cold hands and feet.

Finally, some blood pressure medicines are notorious for triggering significant ED issues as they medicate the same sort of situation I just described. The hypertension drug we recommend patients not use for their blood pressure condition relative to penile performance is Propranolol. We’ve discovered it not only causes ED but also triggers penile fibrosis: the building block of Peyronie’s disease.

FFT is almost identical to the feel of Ellanse in subsequent texture for the first couple of years after your augmentation. In years later as the augmented subcutaneous tissue stretches, it gets a little softer – but never as soft as H/A – unless...a patients receives multiple surgeries of FFT over time. Patients who add 2-3 inches of FFT girth with multiple procedures will often end up with a “squishy” hand when erect.

I’m sure you’re already aware a female partner can’t detect a difference in the rigidity of the penile shaft – they only feel the variation in total measured girth. Most sex toys involving vaginal penetration in today's market have a softer outer finish for improved comfort.

I’m going to provide a very lengthy response to your question in order to help you and others make the best possible decision for yourself in this regard.

Let’s start with the ligaments themselves. Every guy has a different number, size, tautness and exact location of their suspensory ligaments. They were designed into men during a time of evolution when sex between human beings was often a very violent process. Keep in mind: we might like using our sexual organs for fun, but their sole purpose physiologically is the procreation of our species. Many, many, many years ago we needed a lot of structural support at the base of the penis in order to consummate sex. If we had sex like that nowadays we would probably be charged with a crime.

What you “see” of your penis is where the organ becomes visible from underneath the pelvic bone. Your penis organ actually starts near the anus. The suspensory ligaments are connected to the base of the penis from the pelvic bone.

There are actually four aspects to optimal penile lengthening. One, is the cutting of the ligaments at the base of the penis. The second is the release of the deep ligament. The third is repositioning as much of the penis organ that’s situated on the inside to an outward location as is safely possible. And finally, there’s stretching.

It might be helpful to envision the suspensory ligaments at the base of the penis like the base roots you see around a tree in your front yard and the deep ligament as the tap root of that same tree.

I bring this up because we see lots of patients who had a previous ligament release performed and complained they actually “lost” length. How could that happen? It happens because of patients seeking a minimized scar in achieving penile length.

If we only cut the top suspensory ligaments (like the roots around the base of our imaginary tree) – the deep ligament (the tap root) will still hinder the penis from successful stretching. We found this most common for patients who had a ligament release performed through the scrotum to make it a “scar-less” procedure. Patients who had such procedures and are almost always unhappy with their length outcome are sometimes informed it’s likely “the ligament reattached”. However, we’ve discovered that wasn’t the case when we performed a subsequent lengthening surgery. The reason was rooted in the fact the deep ligament held fast and internal scar tissue underneath the skin from the procedure triggered new tautness and a reduction of penile length.

While a VY-plasty is suitable for some patients, we found that we get the most optimal lengthening results via a 2-3-inch singular incision running straight up from the base of the penis at the pelvic bone. This opening allows optimal access the suspensory ligaments, the deep ligament and repositioning of the penis, as able.

It’s important to understand that the penis doesn’t just “jump forward” following a release of the ligaments – they’re not that tight on 99% of patients. That’s why the penile repositioning is so important for most patients – along with the fact it inhibits the likelihood of ligament reattachment. It leaves a scar akin to a hernia operation which is what most single male patients use to describe where they got it in the future.

The degree of work with stretching on the part of the patient following surgery is often under-amplified by some doctors but a crucial aspect of increased penile length. It starts 2-3 weeks after your procedure with a traction device that causes mild tension on the organ while it’s still healing from surgery. At this juncture, we want to inhibit any post-surgical retraction of the penis and loss of any of the hard-earned gains you enjoyed post-surgical. Tissue will naturally retract following any procedure and you want to make sure your penis doesn’t get involved in that process through mild traction. A couple of months later after things are completely healed, you’ll want to evolve to using weights to stretch the penis organ. Results from stretching are no longer inhibited by the strong ligaments you had cut for length. This program will trigger the cells to slightly tear and grow back larger over time – in a process similar to building muscle mass to your biceps. Alas, it’s time consuming. You need to stretch for two hours daily for six months after the procedure to achieve best results.

The fear of a significant change in erect angle while standing up is most often promulgated by doctors who don’t perform penile lengthening procedures. The angle changes 5-20 degree’s depending upon your present angle when standing up straight. It drops more on a percentage basis if your engorgement points near straight upward and less if you’re presently situated me straight ahead. Most men are prone on a bed when erect and never notice that difference.

A bigger consideration for some if the subsequent “looseness” at the base of the shaft following a ligament release. While most sexual positions are still fine in this state, the force of a partner coming down at an angle as with “cowgirl and reverse cowgirl” are challenging without putting your fingers at the base of the penis to add support. Thus, if “rodeo” sex is your favorite pastime, you need to think long and hard before considering a penile lengthening procedure.

Thus, in summary:

1. You must have a ligament release in order to achieve real and lasting increase in penile length.

2. Only cutting the top ligaments to reduce scaring reduces the likelihood your lengthening will succeed. You need the deep ligament cut as well.

3. The post-surgical erect angle isn’t usually a big deal. The subsequent “looseness” at the base of the penis during rigorous sex is a more important consideration for some patients.

4. Stretching is required to achieve optimal results. If you’re not able or willing to invest the time in post-surgical stretching, it’s probably not worth your time and money to invest in a lengthening procedure.


We presently charge $9,800 for our optimal lengthening procedure that includes release of top and deep ligaments plus penile repositioning, anesthesia and supporting stretching devices.

Know that’s a lot of information but I hope it helps you make the best decision for your particular case.

I have been informed by clinics offering this service and other methods of PE, so I presume they are not biased, that about 20-30% of new fat will be reabsorbed in the first 5 weeks whilst everything is healing. To accomadate this fat lost, more fat is injected to allow for this. After the healing time, what fat remains is classified as good fat, becomes tissue with it's own blood supply. This I'm told will remain permanently, and there should be a 20 - 30% girth increase.

However this has now been brought to my attention.

A small amount of fat may survive for a number of years, however, most fat is reabsorbed in 3-12 monthsAn increase in flaccid penis lengthdue to the weight of the fat, making ligamentolysis (penis lengthening surgery) unnecessary.

I'm not going to name the clinics etc, I would prefer to get hard facts which can be backed up or personal experiences.

I think Avanti Derma and Dr C are specialists in their field, and I would have confidence in them. My concern is the product as the UK clinics are very anti PMMA. I feel more confident with the product, I have noted on this forum, many satisfied results, though the individuals do appear to go quiet. I cant work out if this is due to them being satisfied and they have moved on with the lifes, or whether things didnt go so good. I've tried to reach out but I've had no response. The promotion for the upmost package is very tempting. The cost to Mexico is something to factor in, especially as it appears 2/3 visits are required, I'm still uncertain of the safety and long term implications of PMMA.

Fillers, the lack of longevity is a concern. Ellanse E is a new find for me, the longevity of 4 years is appealing. But on the ellanse website it states upto 4 years! Upto is always a big ? There is no mention of suitability of penile argumentation and the longevity for this use. It appears its purpose is for the face. Furthermore one practice has said that have discontinued due to some individuals not getting good results.


I also would gladly share my experience (beyond what's in my thread) in private too if you have any questions. Considering I have 39 cc of product in me, I'm pretty committed at this point. I have messaged 'older' members from the 2.0 forum in the past and after some time received a response. And for the few I have, it's basically been that they moved on in life.

In public though if you want my opinion here: if travel to Mexico is a concern, I personally would be going the Ellanse route and finding the most experienced clinic within reasonable distance of you. And remember with it, the collagen formed may last waaaay longer than the actual 'beads'/product does.
