Unfortunately, it does sound like scar tissue (Peyronie's fibrosis). Please keep in mind that we've been studying and developing our non-surgical treatment of Peyronie's for over 45 years now and have a lot more information on the cause of Peyronie's than most other urologists (Rejuvall was actually born out of Morganstern Health's work in helping our Peyronie's patients get back the size they lost to the disease.). That said, what you're describing does sound like you've got some fibrosis in there.
Fibrosis causes pain only when erect because once all that blood fills the organ, the scar tissue is in a place that once an erection occurs and there is less space in the penis, it presses on surrounding nerve endings, causing the pain you feel.
If you palpate the area of your penis where you experience the most pain when erect, you may actually be able to feel the fibrotic mass, depending on how deep in the organ tissue it is.
This may not necessarily have anything to do with the HA injections themselves, but rather the fact that you probably have/had some wound healing disruptors at play. After having had 4 to 5 rounds of penile filler, and with a cannula, when you pair that trauma to the penile tissue with those wound healing metrics, that is most likely what caused your body to send the scar tissue there to try and repair it.
Unfortunately, without us knowing all the details of your case and doing a full workup including bloodwork, we can't really know for sure how severe your case is or the best way to treat it. Also worth noting as you do your research to find a specialist, please do not let anyone con you into thinking Shockwave therapy or the P-Shot (PRP) will fix it. There is absolutely zero scientific evidence that either of those treatments aids in the removal of fibrosis within the penis. Please don't waste your money on something that won't work.
