Are you saying that the use of Adderall or similar ADHD medicines can permanently cause these negative effects, such as the penis appearing shorter, or that they last for only 30 days after ending taking the ADHD medicine? Hopefully it's the latter.

Also, are there any studies on the differences between different dosages of the medicine?

Thanks!

That’s a fair question. Based on the evidence we’re aware of, we would not tell people that standard prescribed Adderall/amphetamine use has been proven to permanently shorten the penis. The more evidence-based explanation is that stimulant ADHD medications can cause vasoconstriction / peripheral vasculopathy, which can reduce blood flow and make the flaccid penis look smaller, tighter, colder, or more “turtled” while the medication is active. FDA labeling for amphetamine stimulants notes that these vascular symptoms are usually intermittent and mild and generally improve after dose reduction or discontinuation.

So, in plain English: for most men who notice this, the concern is more consistent with a temporary circulation/sympathetic-tone effect, not proven permanent structural shrinkage. There is at least one published report describing amphetamine-related temporary penile shrinkage/perceived shrinkage (“whizz-dick”), but that is not the same thing as evidence that routine therapeutic ADHD treatment permanently damages penile tissue.

I would also avoid giving a hard number like “30 days” as though that is established for everyone. What the labeling and literature support more clearly is that these effects can happen during treatment and often improve after lowering the dose or stopping the medication. I have not seen strong evidence for a universal rule that it lasts exactly 30 days after stopping.

On your question about studies comparing dosages, we have not seen good human studies that directly compare ADHD stimulant doses and then measure penile shrinkage/size changes. That specific evidence base appears to be very limited. What is documented is that some stimulant-related adverse effects involving the penis, especially priapism with methylphenidate products, have been reported after dose increases and sometimes during withdrawal/drug holidays.

So our bottom line would be: temporary flaccid retraction/turtling while on stimulant medication is plausible and consistent with known vascular effects; permanent penile shortening from prescribed ADHD stimulant use has not been well proven in the literature.

And just as an important side note for readers: if anyone on ADHD medication develops persistent genital pain, marked coldness/color change, major erectile changes, or an erection lasting 4+ hours, that is something to discuss with the prescribing clinician promptly, because those are not symptoms to just "wait out."
