Basically the needle can penetrate through the layer of the PMMA or i have to leave some area of the peni without PMMA?

Needle penetrates the PMMA without any problem. (With any luck you will need a longer needle.)

Any information if this would be a risk as to open the PMMA area up for infection?

I just had some acquaintances who had silicone implants which they had to have removed due to a later infection, which they think they got from their frequent boner injections. Maybe the nature of silicone (where the enlargement mass is foreign tissue) versus PMMA (the enlargement is due to body collagen response) would create different risk profiles for this.
