Doc Oates-
Have you tried to use a very flexible silicone sleeve after the procedure to keep the shaft extended for a few days?

I wish Dr. C would consider using Caverject.  Many of my problems are much more visible in an Erect state.  Has anyone asked about this possibility?

Hi Toolman,

No we have never used a silicone sleeve and not sure if we have really seen the need too. But maybe it is a good idea. Not sure where to get something like that either.

Could you briefly outline some of the possible complications that could occur with HA, even when using a micro cannula? Basically what are the risks, other than a poor aesthetic outcome? Having actually felt a micro cannula, I\'m struggling to really see what could go wrong, but at the same time, I\'m aware there are usually possible complications that we as potential patients don\'t really think of.

Thanks

Hi Hoodle10,

The most common is to get a small temporary bruise. We have had 1 guy who had swelling and movement of the product and have had to dissolve some of the HA.

The advantage off the microcannula (it is still 70 - 90mm long, just thin) is reduced chance to get obstruction of a vital structure. Those being the dorsal penile artery and the urethra. Both of these are deeper structures so difficult (but maybe not impossible) structures to get too with a cannula.

There are occasional inflammatory reactions to even HA fillers. More common in people with autoimmune diseases - which are more common in women.

Again the big advantage of HA is the ability to dissolve it if you need too, so even if it is a rare problem there is a quick way of dealing with it.

hi there Dr. Oates,
my question is do u have a a video of your HA procedure, my wife is a physician(practicing over 20 years, 15 of those with injectables) and we have been utilizing PRP the last year or so and have seen some great results with it. we have seen some of the other videos out there and to say at best, they were poorly managed. i would like to c if the combo can make a big difference.
PRP in conjunction with pumping is great for rejuvenation but only has a small increase in size.
thank u...

Hi Sexbeast,

We have recently taken a video and will probably soon have it on YouTube and one of our websites - www.academylaserclinics.com.au

We have also considered doing training courses - I have a history in training and my wife runs the largest truing organisation for therapists/nurses and doctors of cosmetic procedures such as IPL/lasers/injections in Australia. There is a bit more to it than what might be seen in a 2 minute video.

We have considered using PRP but it seems to be of more benefit for the quality of erection rather than increase in size.

AB Silicone has some that are white, and between the yellow and blue ridgetity from Autoextender.  But I\'ve found Autoextender to have better ones as they have three different levels of hardness.  The photo shows the softest kind, yellow.  The blue is the middle rigidity, while the red is the most ridged.

smalljay wrote: Hi Dr. Oates,

I\'m sorry if somebody has asked you this before, but what specific type of HA do you prefer to use on the glans? (Juvederm XC, Juvederm Ultra, Voluma, Restalyne, etc etc etc?)

And what is your technique aka where are the injection points and how deep do you insert the needle, typical # of cc\'s?

Finally I was wondering if you have had any cases of Necrosis from HA to the glans so far that you know of?

Hi Smalljay,

Yes, have gone over this but -
Voluma
I mostly inject around the \'flare\'/corona of the glans. If wanting more volume I inject a second ring inside this or do a \'fanning\' technique.
The needle is in the deep dermis - so about 1 - 1.5mm deep.
2 - 3 cc
I have never had it and never heard of it with HA. HA is softer than most permanent fillers and it can be dissolved. Got a problem? Lets dissolve it straight away. In the face the Necrosis is from injecting into a major artery, in the glans it may be a pressure effect.Too much volume of something too hard.

Hope that helps,

regards

HW,

We would definitely need at least 15ml. That would usually get close to 1 Inch. then we may need to do a second session of 5 - 7 ml. It adds up unfortunately.
