Dr. Oates - I've reviewed numerous of your replies here on PB and I (we) appreciate your participation.I have several questions for you. I didn't want to hijack anyone else's thread so I thought I'd ask them here. I see your clinics are located in Sydney and Perth. Which clinic do you frequent the most? If I were to fly in all the way from the US I'd prefer to be treated by you. Even though I'm sure doctors you've trained are highly trained and very qualified.Do you ever venture out to the US to perform these procedures? Have you ever met Dr. Morganstern? (Atlanta, GA) (just wanted your feedback on options he's offering)Do you believe Ellanse fillers you're using are the same type he is? (Is Ellanse a regulated substance made in a western lab or something that widely differs based on where it was made/purchased?) I'm asking because I noticed some members here chose a '4-year option' filler while others speak of the 'L' version, etc.?Can you explain how the longevity timeframes are derived? Is it based on a concentration of a specific chemical? In other words, the higher the concentration of XYZ, the longer it'll last?...and if that's the case, is there such a thing as having to choose quality (concentration) versus quantity (duration)?...or better yet, is 2-year filler for example of better quality (finer grain, etc.) than a 4-year filler? ..or it ot the other way around?
My understanding is that the finer the particles in the mix the harder/firmer the final product? Basically the more of the tiny 'foreign particles' in the mix that the collagen has to encapsulate, the thicker/firmer the feel? Am I wrong in my assumption? If there's no difference why wouldn't everyone choose the filler with the longest duration?Have you ever used it on patients who've had FFT in the past? (20+ years ago)For someone who's never used Ellanse, would you suggest having a small amount of the filler injected under skin on another part of the body (arm, leg, etc.) to determine whether he or she is allergic to the chemicals in the mixture before going through with the actual procedure? The idea of mixing free-fat with Ellanse worries me BUT I've seen many very positive reviews so far so I try to stay optimistic. I apologize the many questions but I've enjoyed your interaction with members of PhalloBoards and figured you're the person to ask these questions.
Cheers!

Hi - sorry for the delay - have not been on the forum for a while.

Even though I'm sure doctors you've trained are highly trained and very qualified.I really only work from Perth - which is of course even further away. Dr Gavin Scriven in Sydney does even more procedures than I do and I have no doubts about his capabilities.
Do you ever venture out to the US to perform these procedures? No - medical license is too difficult. We are still looking are running training
Have you ever met Dr. Morganstern? (Atlanta, GA) (just wanted your feedback on options he's offering)I have not met him and I have not followed through to exactly what he is doing, it is slightly difficult to know exactly what it is.
Do you believe Ellanse fillers you're using are the same type he is? (Is Ellanse a regulated substance made in a western lab or something that widely differs based on where it was made/purchased?)Ellanse is the same everywhere - it comes in 4 different lasting versions. I was just sent a woman recently with lumpy granuloma in her face from it...
I'm asking because I noticed some members here chose a '4-year option' filler while others speak of the 'L' version, etc.?Can you explain how the longevity timeframes are derived? Is it based on a concentration of a specific chemical? In other words, the higher the concentration of XYZ, the longer it'll last?

The manufacturer formulates it so the Polycaprolactone last 1/2/3/4 yrs. How long the collagen produced then lasts is uncertain.

...and if that's the case, is there such a thing as having to choose quality (concentration) versus quantity (duration)?...or better yet, is 2-year filler for example of better quality (finer grain, etc.) than a 4-year filler? ..or it ot the other way around?

No you just choose 1/2/3/4 yr. The longer lasting ones are more expensive. I have used 4 yr and 2 yr and now waiting to see.

My understanding is that the finer the particles in the mix the harder/firmer the final product? Basically the more of the tiny 'foreign particles' in the mix that the collagen has to encapsulate, the thicker/firmer the feel? Am I wrong in my assumption?

I have not heard that before, so maybe wrong. If there's no difference why wouldn't everyone choose the filler with the longest duration?Price, the \"what if\" factor, what if there is a problem
Have you ever used it on patients who've had FFT in the past? (20+ years ago)No
For someone who's never used Ellanse, would you suggest having a small amount of the filler injected under skin on another part of the body (arm, leg, etc.) to determine whether he or she is allergic to the chemicals in the mixture before going through with the actual procedure?Allergy does not seem to be an issue - but I have seen 2 people with granuloma which are difficult to manage.

The idea of mixing free-fat with Ellanse worries me BUT I've seen many very positive reviews so far so I try to stay optimistic. I apologize the many questions but I've enjoyed your interaction with members of PhalloBoards and figured you're the person to ask these questions.

This is such a new area with lots of variables. There have not been long term high number studies to see what is the best and what are the risks. My son is on ADHD meds and read all the studies for what he gets prescribed. Some times I am shocked how little there is on long term human studies. But much more than we have to work with. I am also surprised how much animal studies go into the safety data of these meds still. We definitely don\'t have that for this work.
Cheers!

Thank you for your detailed responses Dr. Oates.
A follow up to one of your answers - is there a way to minimize the risk of developing a 'granuloma'? Is that basically a lump or an infection of some kind?

Texas - great idea! If I ever decide to go for it I too might be willing to participate in such a study.

Minimizing granuloma risk - I wish I knew. We do it as a sterile procedure. But no-one seems to know why it happens.
