Over time, that tissue can develop fibrosis (scar tissue), nodules, granulomas, excessive firmness, and irregularities.This means that what appears to be a simple lump or uneven area on the surface may actually involve tissue changes beneath the skin.**You cannot correct a structural problem by adding more material on top of it.**Injecting additional PMMA to fill in low spots may temporarily improve the appearance, but it does not remove existing granulomas, correct fibrosis, or resolve underlying tissue restrictions. In most cases, it only makes an eventual repair more complicated.Likewise, steroid injections may help reduce certain inflammatory reactions, but they do not dissolve PMMA.**Why Can't a Surgeon Just Cut Out the Problem Area?**This is one of the most important things we want men to understand.The skin of the penile shaft is already exceptionally thin, mobile, and delicate. When PMMA has become incorporated into the surrounding tissues, especially when excessive amounts (>20mL) were injected or complications have developed, that skin becomes even more fragile, scarred, or tightly attached to the underlying tissue.In these cases, Dr. Carney describes the affected skin as behaving like wet tissue.Making multiple small incisions directly over individual lumps or irregularities will only damage already-compromised skin, create additional scarring, and interfere with healing.For more extensive complications, proper repair usually requires **penile degloving**, a reconstructive surgical approach that allows the surgeon to carefully lift the shaft skin and access the affected tissue without making numerous separate incisions.This provides the access needed to remove or reduce problematic PMMA-containing tissue, address fibrosis and granulomas, release restricted tissue, and preserve as much healthy anatomy as possible.Not every PMMA complication requires this approach. But when it does, attempting to take shortcuts will only make an already difficult situation worse.**This Is Why Your Provider's Qualifications Matter BEFORE You Get PMMA**There is an enormous difference between knowing how to inject filler and knowing how to surgically repair the structures affected when something goes wrong.At Rejuvall, Dr. Kenneth J.
