As a co-author of the Radiesse Bible, “Calcium Hydroxylapatite Soft Tissue Fillers Expert Treatment Techniques”, I would never inject Radiesse or any other non-dissolvable filler into the glans or high risk areas like the nose, temples, and forehead. In addition, Radiesse is simply contraindicated in the lips and under eye. The glans is a highly vascular structure such that it is essentially like a blood vessel and high risk.
The biggest fear with injectables is vascular occlusion (VO) where product is inadvertently injected into an artery thereby compromising delivery of blood, oxygen, and nutrients causing cell death and tissue necrosis due to an obstruction of blood flow. Despite being high risk, I haven’t really seen VO of the glans in my practice possibly due to an abundance of collateral circulation. This means there are other pathways for blood to bypass occluded vessels to continue nourishing the glans tissue.
I typically inject 5-10mL of HA in the glans at a time. One patient of mine really pushed my comfort level as he received 20mL HA in the glans. He is a medical professional so I trusted his ability to self monitor and alert me of any concerns ASAP. He did perfectly fine.
What if a non-dissolvable product is injected into the glans and VO occurs. How does one remedy this? It would require surgery to evacuate the product leaving significant scarring if the glans can actually be saved.
This reasoning is why I do not advise injection of irreversible products into the glans. Some of my fellow co-authors of the Radiesse Bible do inject Radiesse into high risk areas like the nose, temples, and forehead. I just don’t have that comfort level especially when there are reversible products capable of doing the same job with reasonable longevity.
Glans filler with HA is a well documented treatment for premature ejaculation. Perlane (now known as Restylane Lyft) was used in one study and demonstrated efficacy even 5yrs out. This longevity, however, is independent of girth enhancement since the study did not monitor this parameter. Nonetheless, benefit is imparted by presence of residual HA filler.
I have before and after photos of glans filler. However, there is never an immediately after injection photo since the glans is very bloody. Instead, the after photos are when they return for a top up several months after. There is no telling what the initial girth measurement is compared to how much filler remains. I will not have my patients return 2 weeks after augmentation and induce an erection just for measurements.
FYI, current pricing on glans filler is $1250CAD for every 5mL after shaft filler. If someone comes in specifically for glans filler only, the starting 5mL of HA is $2500CAD, each additional 5mL is $1250CAD
