Hey guys, I recently turned 22 and I’m from Australia. I’ve just found out that my penis Girth is well below average. I’m currently measuring about 4.25 inches in Girth, which has made me very anxious about my future. I already struggle with some body dysmorphia because of my height (5'7"), and now this is another insecurity that’s really hit me hard.

After doing a lot of research, I’ve been considering the filler route. However, I have serious concerns about the long-term effects of fillers. Almost every thread I read where someone gets fillers, the OP eventually disappears and never comes back to provide updates. This really worries me.

I’m especially concerned about PMMA, since it seems very difficult to remove if complications arise in the long term. I haven’t seen many people with 10+ years of experience with fillers, though I could be wrong. Is there anyone on this forum who has continued to update people about their life after getting fillers? I’m desperate to read any kind of long-term experience or hope. Honestly, I just want to reach 5 inches of Girth, and I think I’d be content with that.

Do you think I should wait another 7–8 years for the technology to improve? I was planning to get married around 30, so I’m trying to think long term. Also, if someone develops issues like ED after fillers, how is that usually managed? Is there anyone here who has personal experience with this?

PS: My Erect length is 6 inches and ive gone to 6.5 in a leluv pump too.

I'm so sorry to hear you're struggling with all that, but the good news is your girth is actually right about at average, not "well below" as you stated. BJU International did a systematic review of several studies on penis size and found that the average erect girth is actually 4.59 inches: pubmed.ncbi.nlm.nih.gov/25487360/

I’m especially concerned about PMMA, since it seems very difficult to remove if complications arise in the long term. If you do decide to have PMMA girth enhancement, we recommend going to a urology-trained provider who has experience and training performing these procedures and has a deep understanding of penile anatomy and tissue behavior. I would also caution you to research which brand of PMMA is approved in whichever country you decide to have the procedure done in, then when you choose a provider ask which brand of PMMA they use, ask them to show you the unopened packaging before injecting you, as well as the lot number/traceability. If they can't or won't give you that information, that is a huge red flag. There are more and more penile injectors out there who are telling their patients it is PMMA when it is actually a liquid silicone concoction, which is extremely dangerous because after a while it easily migrates to other organs and can cause many other serious complications that require extensive surgery to address.

Also, if someone develops issues like ED after fillers, how is that usually managed?

One of the reasons we recommend urologic care when it comes to girth enhancement is that whichever provider you choose needs to be capable of handling any complications that come up afterward, since they are already intimately familiar with your case. In our clinic, we treat the root cause of the ED, whereas many others will just throw a pill at it, which may not be what you need in your specific case.

Best of luck to you whatever you decide, and please let us know if you have any more questions.

Thank you for the detailed reply doc.

My main question was should I wait 8 years / celibacy until marriage /wait for technology to evolve? and do i start with HA fillers or PMMA if I decide to start today?

If I was your son, what would you advise me? considering all the knowledge and expertise you have. Thank you

Unfortunately, that is a decision you will have to make for yourself. Before you start researching providers, I would take a long, hard look at why you're considering Girth enhancement and the psychological factors behind that. How will you feel if you end up not liking the result? What if complications arise? Will you truly be happy with the result if you reach your size goal? These are all questions any man should ask himself before "pulling the trigger" on any kind of Male Enhancement procedure. I wish you the best of luck on this journey.

I'll be quite frank because I can relate to how it felt around the same age with similar issues surrounding self-image. This is a long response, so I'll break them down into sections that are underlined.

Age and If-&-When To Consider
I want to first stress that you are young, so much so that I'm personally of the belief that guys should not be considering any sort of elective procedure of this kind until AT LEAST 25, and more ideally at 30.

The reason it's better to wait it out is because of maturity + financial stability.

The former (i.e. maturity) relates to how your perceptions of self-image, sex, etc, are all going to evolve and change in the next 10 years. You may find you have a partner who is quite content with your size. Or possibly you might find that you put more weight on the issue when you were younger because of internal conflicts that have now resolved, or external influences that no longer bother you... or none of those come to fruition, and you could find that this feeling still persists. But at least by then you'll have given yourself the time to learn more about love & life before making decisions like these.

The latter (financial stability) is about how "saving up" for one procedure is not the optimal place to be in -- these procedures may require follow-ups, which could be for aesthetics or additional gains, and that doesn't even take into account travel & logistics, much less complication management (since no procedure is without risk). If you aren't categorized as someone with disposable income, or someone who can take on a small loan without financial setback, then I wouldn't consider you categorized as someone ready to undergo these procedures.

As for whether or not you should wait, yes I would recommend waiting, but for the reasons I listed above and not because of some unforeseen advancement in penis enlargement. Quite frankly, Research & Development (R&D) in male Phalloplasty is lacking, and most innovations are being made at the leading Clinics from around the world. This is likely due to how purely elective penis enlargement is (versus other phallic conditions in which a medical condition can be cited to justify investment into its treatments, like Peyronie's Disease or Erectile Dysfunction).

And now that injectable Phalloplasty has gained so much traction in recent years (which is the only reason why it's becoming more acceptable in recent times... prior iterations involved unrefined methods full of botcheries and severe complications), there is little incentive to create a brand new procedure for Girth advancement. The efficacy has been demonstrably acceptable to satisfactory, and in some cases stellar (under the assumption that it's performed by a qualified practitioner), to the extent that any refinement beyond existing methodology is largely unwarranted… at least as I see it.

Long-term Feedback from Patients
As for long-term reports, guys here do update often, but the majority of those updates tend to be in the early stages of their Progress Reports. This is because most changes are happening immediately post-op, and you will find that guys post less often (or farther apart) as years go on because there isn't much to add... once they've gotten what they came here for, many will sail off into the sunset. I will continue to ask sincerely that anyone reading this who intends on Progress Reporting to please provide at least an annual update if possible -- it really helps put others at ease and pays forward what the men did before them (and encourages more guys to uphold the practice of routine updates, even if just annual).

What I can say is that some men still do in fact chime in, years after the fact. I myself had my first injections in 2011 and my last around 2015-ish. I am in touch with men who don't post much (or at all) who have expressed satisfaction a decade+ out.

PMMA and Australia
As for PMMA -- I am not sure if it is approved or available in Australia at present time, and costs are going to become exponential if a 22 year old wants to fly to the States a number of times over the course of however long it takes to attain their desired goals. By that I mean, you may want to at also give Hyaluronic Acid (HA) consideration, its lack of permanency would be more beneficial if you felt adamant you were going to pursue penile augmentation prior to 25/30 years of age. Regardless of filler, no procedure is without risk.

I should mention that I'm in talks with a potential Australian Sponsor listing, so stay tuned and maybe I can find a Provider that you can at least consult with, so that in your late 20s/early 30s (when you get your bearings) you'll be in a better position to make a decision on this matter.

Height
Don’t be so hard on yourself... your height is actually the worldwide average for males; even if where you live your height is considered below average, that doesn’t make you below average as a person. I’ve seen one too many “short kings,” as they’re called nowadays, pull quality partners on countless occasions for me to believe height alone is some kind of universal disqualifier (now don’t get me wrong -- in some arenas, like online dating apps, things can be a bit more superficial or shallow, though those are quickly becoming obsolete). There are some women who do have a height preference, but then again, remind yourself that you, too, have beauty preferences about women -- and that those without your preferred traits are no less of a woman.

I would like to end on some small bits of advice that can help alleviate some early adult-life dysmorphia, especially for young men:
[1] Discontinue or minimize adult content consumption (i.e. pornography). They often portray well-endowed men who can seemingly stay Erect indefinitely, shot at angles that over-exaggerate the importance or necessity of size.

[2] Discontinue or minimize the consumption of online forums that deal with penis size (including the PhalloBoards), because men who are larger are disproportionally more likely to post (and on some other sites I won't mention, even use it to stroke their own ego) -- this disproportion inadvertently makes the above-average seem average, which can skews one perception of what "normal" or "common" really is with respect to penis size.

[3] This might be a shocker for some given my role here, but my contention is that size is kind of* overrated. I put an asterisks there because I want to clarify that this remark isn't a sweeping one (i.e. "all instances"), but still relatively true in general.

What I mean is that as I've gotten older, and have experienced every condom-bracket from Snugger Fit, to Regular, to Magnum, to Magnum XL... I've come to learn that penis size is not nearly as high on any sane, mature, and normal woman's preference or priority list when it comes to a man, and that if anything, we seem to care more about it than they do. Authentic size queens are a vocal minority, and any woman who uses your size against you is being cruel (or is leveraging in a relationship in which a guy has an inflated ego, but that doesn't make it acceptable regardless).

Granted, for homosexual men I can't speak for, and admittedly, they do face a bit of a tougher and more competitive sex-life scene based on what I've learned here and among friends. My generalizations are in the context of heterosexual relations.

All in all, I assert size is overrated because being a man is a total package, not "just your package." An initial physical attraction, a good sense of humor, intelligence, career-focus, education, values (moral and ethical codes), financial stability, familial bonds, kindness (not to be confused with "niceness"), sense of fashion, and self-maintenance (health/fitness, hygiene, & grooming) are among a variety of factors that make up each man, with each of us scoring more points in some areas than others. Then you have additional check-boxes like "well traveled, hobby-oriented, and affectionate/attentive" that might make some women's preferred lists... and I'm willing to bet sheer penis size alone probably won't crack the top 5, or even top 10 of the majority of quality (worthwhile) women, and if anything, it might be something more about "having a good lover" than it would be about length & Girth.

So why get penis enlargement if I think it's overrated? Well for starters, saying something is overrated doesn't mean it can't be worthwhile in some instances. However, my real answer is simple: it adds points to one of many aspects of yourself you wish to self-improve, like the attributes I listed in the previous paragraph. Why do people hit the gym if they are strong enough to carry most things in their life? Do you think 99% of human needs to bench more than 200lbs? Dating has become increasingly competitive, and some guys find that, by resolving their “hang-ups,” they can in turn improve their self-confidence... probably the most attractive thing about a man to women (and not to be conflated with arrogance).

Good luck!
