I would like to ask Dr. Liu a question, although anyone who knows can answer me.
I read on his website that gains made with an extender after cutting the ligament were more likely to be permanent compared to gains without cutting the ligament.
My question is why is this happening? Thank you in advance for your attention.

@KV87 Dr. Liu probably has better response but having had the lig cut procedure myself with Dr. Liu my understanding is that when the ligaments get stretched they are very good at returning back to [near] their original state. When most of the ligaments are cut this obviously cannot happen.

On a side note the reason Dr. Liu recommends stretching/hanging after the lig cut procedure is to prevent the formation of permanent scar tissue, which can also cause shortening.

Hi KV87,
My apologies for the delayed response. I have been away and was unaware of the ongoing discussion until PhalloBoards notified me.

When the suspensory ligament is divided, a gap forms between the severed ends. If left untreated, these ends will naturally reconnect through scar tissue formation. To prevent this, we introduce a barrier into the space to keep the ends from reattaching. This is achieved by repositioning adjacent tissue—such as muscle, fat, or fascia—and suturing it in place for two key reasons:

To prevent the cut ends of the ligament from reattaching.
To reinforce the area, providing a strong foundation for support.
This method is generally effective, and reattachment of the ligament is rare. However, while the ligament ends are prevented from reconnecting, they may adhere to surrounding structures. To mitigate this, it is important to keep the penis in an elongated position post-surgery so that if any adhesion occurs, it does so in the desired lengthened state.

Post-operative care is crucial. Manual stretching of the penile shaft and the use of stretching devices help to maximize and maintain elongation.

The purpose of stretching after ligament division is to maintain the penis in an elongated position while healing, ensuring long-term results. In contrast, non-surgical stretching temporarily elongates the ligament, but once stretching stops, the ligament naturally retracts to its original length. This is why stretching alone, without surgery, typically yields only temporary results.

The penis itself, when erected, is at its maximum length and is very difficult to be made longer safely. What is done safely to make it "longer" is by re-positioning it so that more of the penis shaft is visible and "usable." This is done by detaching the superficial and deep suspensory ligaments and any other attachments AND securing it in position so that the penis is stable and strong. In addition, fillers can also be used to enlarge the penis shaft and therefore it becomes longer as well. Different surgeons call their procedures different names but basically, this is the way it is done. So it is difficult for the operation to change the Erect length, but could it increase with prolonged use of an extender post surgery or is there only Flaccid increase
