I will be getting a filler in the next few months. I’m unsure what filler I want. However, I’m 99% sure it will be done at Avanti. I’m 41, Circumcised, happily married with 2 kids. HA is intriguing for the reversibility but I have read reviews that it dosent feel natural and it’s “squishy”. When it comes to PMMA vs Ellanse, the reports that Ellanse is last much, much longer than reported makes me wonder what’s the difference between PMMA and Ellanse. Cost isn’t a factor, know it’s a step wise process and ready for multiple procedures. Any opinions

Neither technically feel natural but most often times (like 99% of the time it seems) only the person with the enhancement notices this, and if you're married, I doubt either filler will make a significant impact in that regard. I believe the palpability of Hyaluronic Acid (HA) is overblown and I know of many reviews that are quite content with the overall feel and Girth of HA.

The main take away here is that HA will require top-offs periodically (the frequency will vary person to person) to retain size, but comes with a strong safety profile (HA is naturally occurring in the body and reversible to some extent) due to the fact that if you don't like it, you don't have to worry about it lasting forever. PMMA on the other hand will require a set-amount of appointments, and then you'll be set for life; however, in the (uncommon) event of complications, anything permanent will prove to be more challenging to manage.

You are correct about your assessment on Ellanse. As for the difference between Ellanse & PMMA --> we know that PMMA is permanent, and we are told that Ellanse is temporary, however most (if not all) reports of Ellanse Girth enhancement don't appear to show any significant reduction in Girth, which means it's very hard to put a timeframe on its longevity.

Between the three, there are no wrong decisions, especially if cost isn't a factor. If you are set on Avanti Derma, then honestly I'd say do PMMA or Ellanse, because that's definitely their strength. You can do HA with them as well, but with HA you are afforded a number of similarly talented practitioners around the U.S., whereas quality PMMA practitioners are far & few (not all fillers are created equal). Ellanse can only be done in Mexico or Europe.

I think at this time I’m leaning towards Ellanse at Avanti. Will have a consult with Dr. Morales shortly to discuss further. If the 2 year version lasts 5 or 6 years it’s great as it will go away at some point, worst case. Also SD is an hour flight for me compared to seeing Carney in ATL. I’m not sure I want to have a procedure every 8-12 months line I would likely need with HA. It’s a tough decision

Just to clear something up with HA -- some men can last much longer than 12 months before requiring top-offs, you really won't know the rate at which your body absorbs until you get it; however, more importantly, when you go for your top-offs, you are simply adding some of what was lost, and NOT starting from scratch all over again. Suppose you go from 5" to 6" via HA. Suppose after 15 months you find that you are at 5.5". This means your follow-up appointment only requires enough volume to get you a gain of 0.5" and not a whole 1 inch, likely improving time (on the operation table) & costs (price-per-cc) overall.

Furthermore, PMMA and Ellanse will almost certainly have anywhere from 2 to 4 follow-up rounds to get to your desired goals & aesthetics, meaning you are already committing to a year+ long commitment with multiple trips. The difference of course is that HA will be a lifetime commitment whereas PMMA & Ellanse not so much.

I think these facts are overlooked which is why I wanted to clarify. As for your decision, I'd definitely go through with your consultation with Dr. Morales, she was trained by Dr. Casavantes (the godfather of non-surgical male phalloplasty). As for Dr. Carney, he no longer offers Ellanse but can perform HA and PMMA (Bellafill PMMA, not the Mexican iteration). And if it was HA exclusively, you could visit the North American Directory if you want to limit your travel time.

As I said before, no decision between the three are wrong, so long as your practitioner is qualified & experienced. Good luck!

To add my 2cents to the pro's and con's of HA... My H.A has lasted 3.5+ years, unfortunately the results were terrible and as I posted before it has taken over 2 years of hyalase sessions to get rid of just some of it (although I'm told I'm an anomaly). So there is the possibility of HA lasting a long time which could be good if you got good results.

I had 4-year version of Ellanse exactly 4 years and 3 months ago. I had 12ml and my EL is 6.0". I went from below average (4.6") Girth to 5.1", exactly 0.5" gain which has not changed since then. Very satisfied, texture 100 percent natural flaacid and Erect. I did have a lot of inflammation. At the time, Dr. Kearny suggested two sessions of 10ml due to inflammation and I opted for 12 the first session... I ended up with two nodules/fibrossis, one went away the other dimminished (I can still feel it but it doesn't bother me). That was the only reason I didn't go back for the 2nd round of 8ml (it was refunded). Happy with the 0.5", didn't want to push it.

Overall, I'd do it again as soon as I notice any decrease, and just emailed Avanti with some questions and to plan in advance, since Rejuvall no longer offers it in the states.

One of the questions I asked them is if they had any data from their patients on how long after the official "shelf life" (4 year in my case) that they started noticing any decrease, and if they felt there was any residual "permanent" gain from it. In any case, I'd do it again, and probably will.

EDIT: Only the 2 year version is available now. It is about 500 per ml per email from Avanti. They also said it lasts longer than what is stated by the manufacturer, which I'm obviously observing. Time will tell how long.
