Background:
1. I used PeniMaster Pro which did help increase my length overtime (about 15cm BPEL to 17cm BPEL, in under 400 hours, so I will still keep going). I didn't see much Girth change.
2. I did use a LA pump and the Girth was good but no real gains over time. My base Girth has always been pretty okay (over 5 inches, stable at about 5.25 inches currently).
3. I got some Juverderm HA filler and some cheaper filler. The first batch did thicken the penis nicely, but the filler was 'soft' and the top of the penis felt rather spongey. The weight of the filler also caused Erection issues. The Girth was 6.5 inches and way too thick for my old length so I dissolved some of the filler so that EQ would improve and it would look more aesthetic.
4. I then got some yearly top ups with Juverderm Voluma (max 2-4ml at a time) to keep the Girth at around 5.75-6 inches which was very comfortable during sex and otherwise.

What I am looking for:
I am very fit and active and therefore the HA filler breaks down rather fast (2ml will pretty much absorb back within 6 months) and topping up is becoming cumbersome and expensive. I've been exploring options. I am quite happy with my length, will still use PenisMaster Pro to maybe get 0.5 inches more if possible, but not a focus. I would like to get to about 6 inches Girth and maintain that but not have EQ issues and for it to not feel "soft". Generally I've had no issues with the fillers (lumps etc all settle within a week or so). I work from home so can bandage / take care of it but want suggestions on what is best. I want to avoid surgical options and I am based in New York.

Options (all are affordable for me):

1. medmancave.com -- they claim a permanent filler and a gain of 0.75-1.25 inches. I tried digging into what exactly they are doing, they claim its not injecting fillers exactly, but some silicon thing which is drained out by the body within 30 days, and collagen will be left underneath. Seems no sex for ~30 days.

2. Bellafill or some more permanent type filler -- this seems like a good option given I've been using Voluma with no issues etc and seems more permanent (although, I read Bellafill is ~5 years, so unsure how this is permanent). Seems no sex for ~1 week which is fine.

What do you recommend? Doctor/clinic recommendations (ideally in NY) are welcome!

After some initial impressions I didn't see what kind of filler that Medmancave place offers. If they claim its permanent, it's likely Bellafill. The only other permanent dermal filler is silicone, which I would NOT recommend. I'll be curious to know what it is that they inject, I really wish these Clinics would be more transparent with that information upfront. If it was somewhere on their site, then that's my bad. I'll reach out to them this week and do a little more info-gathering.

Also, while I've never heard of the "weight" of a filler affecting erections, you'll find that any option will have this issue, not just HA.

Bellafill (or any medical grade brand of dermal filler PMMA) is absolutely permanent.

My opinion is to either stick with HA considering its relatively strong safety profile, or if you prefer the cost savings of a permanent option, PMMA is the way to go. Some Sponsors here offer PMMA as an option, but the ones that specialize in its use are:
Avanti Derma (Mexico)
Youthology Med Spa (Ohio)

Bellafill is ~5 years, not permanent from what I have researched?

PMMA - is this fat transfer? I am not very keen on this.

I spoke with Medical Man Cave about 6 months ago. After a little prodding they confirmed that they use Silikon oil, same as Loria. Makes sense, they use the same pricing structure.

Is Silikon oil a bad thing? This is what Medical Man Cave said to me:

Our procedure produces permanent results. No need to come back in for top ups. We do not use a permanent filler. The oil based silicon that we use is replaced with naturally produced Collagen. After 30 day the oil based silicon will be flushed out of your body, leaving you with only the naturally produced collagen. This is what produces the permanent gain in girth. The procedure will not affect your ability to get an erection or change any level of sensation in the penis. Everything will look even , there will not be a spongy or lumpy feel or look to your penis. After the collagen fully forms, you will see an increase of about ¾” in girth and you may see a small increase in length at the head of the penis. The shaft and head of the penis will look even, due to the even distribution of the injected product and collagen growth.

Silicone!? Every doctor strongly advices against it, saying it is a very outdated method, and I saw no clinic anywhere in the world offering It. I read initially the results can be good, but it seems in almost every case it will be disastrous after some time. The only thing worse would be petrolatum, I believe.

This is unfortunate. I was hoping we could have a Colorado-based Sponsor here but upon learning this I'm afraid I won't be reaching out.

Silicone oil is deceptive in that it can certainly provide instantaneous and aesthetically-pleasing Girth, but can come with serious issues if & when complications arise. Also, given all the other relatively safer filler options available (PMMA, Ellanse, HA, Radiesse, Renuva, and Fat), why even play with fire??

And as far as Mancave's assertion that the silicone would be "replaced" with you collagen, that's simply untrue. Collagen may develop, sure, but the silicone isn't going anywhere and will remain in your penis. I'm troubled they are being so inaccurate here.

That thing about the silicone being "flushed" out of the body is intriguing, but totally unbelievable. At the same time, it makes me wonder if anyone came across such claims before?

Here we have someone claiming that the silicone will be "replaced" by collagen, and the silicone will be flushed out of the body. As far as I know, silicone is one of the substances that are likely to be able to transition elsewhere in the body, but it cannot leave the body.

Silicone breast implants vs saline implants are a perfect example. If the saline implant is ruptured, it will indeed leave the body, but this is absolutely not the case with silicone implants. Because of how common silicone implants are, and having been around so long, it is well-known how silicone reacts in the body.

PMMA are microspheres of a plastic-like material that are suspended in carrier material (cellulose or bovine), and injected into your skin to induce collagen growth, which in turn yields volume (hence Girth enlargement). Bellafill is the American (patented) brand of PMMA. A PMMA filler is absolutely permanent and I'm not sure why any Clinic would simply claim "long-term" as opposed to "forever" but it may be a wordplay thing, who knows.

I have a consult with Dr Loria and he does inject small droplets of silicon which then cause your own collagen formation etc. There are a lot of positive reviews. The negative ones tend to be for guys who want a "monster" size which is not my goal. I am tempted by this as we are both in Miami so I can pop by his office anytime for help vs Avanti Derma which is a flight away.

I looked into PMMA also and the clinic (Avanti Derma) claimed it will last 5 years or so ( drive.google.com/file/d/14SDp6hLDu08iyoIGuJgsfFU1bfMiEMoU/view ).

Ellance ( phalloboards.info/general-forum/progress...inside.html?start=15 ) doesn't seem to be a bad option also as I'm happy to top up after 5 years or so (just tired of the yearly top up).

I have been using 2-4ml Voluma yearly as a top up to keep my girth but its just too soft and I don't like it. Thicker is nice but only is "hard" all over without lumps and bumps.

My erect girth is around 5.25" and I only want to get to about 6" (even a bit less like 5.7" is fine). My erect BPEL is just over 6.5" which I'm happy with.
I am gay so a very thick penis is no good as the guy won't be able to suck me / will be hard to penetrate then. I'm not just trying to round it out so its a nicer looking dick / cosmetic rather than the thickest one out there.

Any suggestions are welcome on what is best here.

A few corrections here - PMMA is permanent and therefore lifetime. If travel is an issue, Dr. Rupeka and Dr. Carney also perform it in the states.

Ellanse was projected 1-2 years by the manufacturer but Clinics are reporting much longer lifespans (3-4+ years). Ellanse is only available abroad.

Hyaluronic Acid (HA) may not be long-term, but the trade off is the best safety profile among the fillers mentioned here.

I would not recommend silicone oil. After seeing Dr. Loria results and how the patients describe their procedure, I'm reluctant to believe he uses a micro-droplet technique. Of all the Doctors discussed on this site (who happen to be innovators in the field of male enhancement), none of them employ silicone oil for girth despite having access to the filler....begs the question, why don't they also use it? This is because if & when complications from silicone oil happen, they can be incredibly difficult to manage. Dr. Loria himself would be unqualified to perform any surgical intervention in the event you need your penis saved and it will likely come out of pocket. Admittedly, silicone results can look pretty good immediately post-op, but the amount of volume Dr. Loria uses would have me a nervous wreck with all that inside me.

There is also Renuva used by Dr. Tsay which is basically like a fat filler but without the need to harvest it surgically, making it a relatively safe option as well.

If all you need is 0.75" to reach your goal (which is a reasonable one), any of the aforementioned options (minus silicone) should work. You'll want an experienced practitioner. Good luck!
