Hi All:

I'm contemplating have a second lengthening surgery. I've had a a couple rounds of PMMA done and then finished with a lengthening surgery a few years ago. While the lengthening surgery went well at first, very shortly after it I could feel my ligament reattach, and over time I developed a large ball of scar tissue formation in my pubic area which fully reattached the penis to the bone. (Yes, I stretched as much as possible afterward). I believe that the reattachment was due to either PMMA migration to the pubic area, testosterone, or both. The physician at the time did not use any sort of spacer either, so that may have sped up reattachment as well. The length I originally gained from the surgery was lost, but I'm generally now where I was at before, except that I have a very, very big pad of fat and scar tissue that makes my penis look small and causes large amounts of discomfort on a daily basis--the fat pad/scar tissue is around 4 inches thick at this point.

Has anyone had experience with this issue in the past and removing the ball/pad? I've spoken with two different physicians about it who are both leaders in this field, one of whom said he was not concerned about further length loss and reattachment if I were to have a second surgery with a spacer and follow after care instructions; the other seemed to feel the exact opposite and did have such concerns.

Lastly, I'd be curious to know if anyone has any data or thoughts on the effects of testosterone on scar tissue formation. I'm currently on testosterone replacement therapy, and given that testosterone increases tissue formation, my thought was that having high levels of testosterone could increase the rate and amount at which scar tissue forms after a surgery, which may have been a contributing factor to the original reattachment. I tried to do some online research on this but couldn't find much.

Any thoughts would be appreciated from those with experience or actual expertise.

I have been on TRT for 16 years. In that time I’ve had 10 or so surgeries and went through chemo for 2 years for stage IV cancer. I have had multiple comments from different doctors about how well my scars healed. Everyone heals differently and in my experience, TRT has not negatively affected my scar healing.

Thanks for that feedback. Were any of the surgeries PE related? I think the immediate healing is sort of my issue: with a ligament surgery, you want time for the stretching to take place before the ligament heals itself and reattaches to the pubic bone. So being on TRT can actually cause you to heal faster than anticipated and prevent the ability to stretch, possibly causing further contraction. Just a thought though. Not sure if there's any evidence or science to it.

3 surgeries were PE related. The initial Elist implant surgery. The implant Removal. Both in 2010. Then last month, my 3rd PE surgery. On my last surgery, the Dr had me do gentle, manual stretching immediately and starting about 10 days after surgery, I began ADS.
