ed doctors can do revisionist circumcisions to make them tighter. I'm not sure why the doctors you were seeing were saying only a turkey neck procedure would make it tighter. This doesn't have anything to do with a circumcision. Also,

Whether you have PMMA in your dick or not should make no difference as to the circumcision being carried. The doctor will measure tightness accordingly if you find one who is experienced in this. Urologists who do standard circumcisions for patients with phimosis don't practice cosmetic circs. I learned that going to a random hospital and talking to a uroligist whom you have no idea what their experience with circs is will not provide you the answers you're looking for.
UPDATE SINCE THREAD POST:
I saw Dr. Cornell in Atlanta and am currently healing. The MARTA system from Alanta airport takes you two blocks away from clinic. It cost $6 and trains come like every 10 minutes. I literally walked straight off the plane into the train terminal and a train was there in 1 minute. I'm hoping he didn't cut me too high. Here's what happened. I told him I was considering Phalloplasty and he, like most doctors, strongly advised against it. He's seen all the horror stories from the past. He's not aware of the current filler procedures but even if he was he'd still advise against it. Very few urologists will recommend it.
So, I told him low and tight is best for fillers and he quickly understood and knew why. We were looking at my shaft and I was trying to determine where the lowest cut possible would be just for my own cuosity, I didn't want the maximized low cut I see some guys get (there's a thread on Reddit dedicated to low cuts). Removing the entire foreskin just seemed unappealing to me and I feel like even if it meant the filler would be more uniform, it would creep over my glans due to the lack of separation.
I'm at 5.25 mseg if I masturabte daily. I haven't measured yet when I abstain but because of the very few past experiences where I dd refrain from daily masturbation, I figure I might be 5.35-5.4 mseg when at full max turn on. My point is, I don't "need" phalloplasty. So when you're with a urologist and they tell you the same thing, you can very easily agree with them. I told him I wouldn't go through with it and during that time in his office I did feel like I shouldn't go through with any fillers.
So, as I was asking him where the lowest cut possible would be (I don't think he fully realized I was just curious) he suggested to go for something that'll look better and he slid his finger down a little bit and suggested putting the scar there. He said it was the best spot to put the scar. It didn't look too high at the time when he showed me with his finger. I'm really interested in seeing where it ends up.
Here's the other thing that I took away. There are other factors he was considering when deciding where to place it. He saw things in my anatomy that told him where to put it. There were natural markers he could see telling him where the best cut would be for both appearence and erectile comfort knowing I wanted it tight. Before he cut, he stretched my shaft and took a little time to figure it out. I heard him say, "there we go, there's the sweet spot."
This is what I'm concluding based on this experience so far on day 5 of healing (I haven't seen my dick yet since before the procedure): Ian told me me low and tight is ideal but he followed up with saying, "but see what you're doctor says about where best to put the scar". I'm glad he said that because that's exactly what happened with Dr. Cornell. He saw what would be best. I didn't feel like I should step in and say, "no, I want it lower". I've seen guys with not real low circs and their PMMA results are fine. I can't get back the foreskin but I can always cut more. I didn't want to go against the advice of one of the best circ doctors in the US on where the ideal cut line would be. Again, having not seen it, if it's real high I may be perturbed. But that'd be my fault for nt communicating with him better. I get the feeling it's not but I don't know.
I read someone post saying, "don't pay extra for a doctor who specializes in circs". I couldn't disagree more. If you ask a standard urologist about low vs. high, tight vs. loose, they won't know really what you're talking about. They aren't trained to do that and don't use that terminology. You will make them nervous that they won't give you the result you're looking for.
